#12 Authorize to enter into an agreement with JSI Research & Training Institute Inc., d/b/a Community Health Institute, Bow, NH, to provide professional support services to the Department’s Oral Health Program in order to implement activities funded through federal grants, including access to preventive and reparative dental treatment for individuals in the statewide community-based and school-based oral health programs; conducting dental screenings, fluoride treatment and referrals for treatment in substance abuse treatment settings; as well as workforce development activities, in an amount not to exceed $2,605,345. #14 Authorize to amend an existing contract with Community Action Program Belknap-Merrimack Counties Inc., Concord, NH (originally approved by G&C on 6-21-17, item #49), for the provisions of a Commodity Supplemental Food Program and a Senior Farmers’ Market Nutrition Program statewide, by decreasing the price by $170,707 from $1,680,610 to $1,509,903 with no change in the completion date of September 30, 2021. 100% Federal Funds.

#14A Authorize the Division of Client Services to exercise a renewal option with University of Massachusetts Medical School, Shrewsbury, MA (originally approved by G&C on 10-26-16, item #11), for the provision of Disability Determination services, by increasing the price by $302,640 from $778,250 to an amount not to exceed $1,080,890, and by extending the completion date from June 30, 2019 to June 30, 2020. 75% Federal, 25% General Funds.

#15 Authorize to exercise a retroactive renewal option to a sole source agreement with Friends of Recovery, NH d/b/a Hope for NH Recovery, Manchester, NH (originally approved by G&C on 3-7-18, item #16B), to continue the provision of Peer Recovery Support Services, by extending the completion date from February 28, 2019 to June 30, 2019, with no change to the price of $600,000.

#16 Authorize the Bureau of Drug and Alcohol Services to amend an existing agreement with the Prevention of Certification Board of NH, Bow, NH (originally approved by G&C on 10-7-15, item #20A), relative to the provision of the Prevention Specialist Mentorship Program, by decreasing the funding for State Fiscal Year 2019 by $7,693, thereby reducing the overall price $88,000 to $80,307. Effective upon G&C approval through June 30, 2019. (2)Further authorize to exercise a renewal option with the Prevention Certification Board of NH, Bow, NH (originally approved by G&C on 10-7-15, item #20A), relative to the provision of the Prevention Specialist Mentorship Program, by increasing the price from $80,307 to $92,307 and extending the completion date from June 30, 2019 to June 30, 2020. 85.10% Federal, 13% Other, 1.90% General Funds.

Division of Long Term Supports and Services
#17 Authorize to enter into a sole source amendment and exercise a renewal option to an existing agreement with Navigant Consulting Inc., Chicago, IL (originally approved by G&C on 5-17-17, item #12B), to assess, analyze and explore alternatives to the current service delivery model for long term supports and services and make recommendations to guide the transition to a future service delivery model for the Bureau of Elderly and Adult Services, by increasing the price by $550,000 from $204,300 to $754,300, and by extending the completion date from June 30, 2019 to June 30, 2020. 86.46% Federal, 13.54% General Funds.

Division of Economic & Housing Stability
#18 Authorize to accept and expend the Emergency Solutions Grants Program funds under Subtitle B of Title IV of the McKinney-Vento Homeless Assistance Act in the amount of $39,610. Effective upon G&C approval through June 30, 2019.

DEPARTMENT OF TRANSPORTATION
#19 Authorize the Bureau of Right of Way to transfer a 6,160 +/- square foot parcel of State-owned property located in the Town of Londonderry, situated between the north side of Roundstone Drive and the south side of Old Pettingill Road to Hazelton Pettingill LLC. This transaction is in exchange for the transfer of an 11,828 +/- square foot parcel of land from Hazelton Pettingill LLC to the Department of Transportation to be used as an Access Road. (2)Further authorize to waive the Administrative Fee of $1,100. Effective upon G&C approval. No Cost Transaction.

#20 Authorize the Division of Operations to transfer $4,646,400 from the Highway Surplus Account to fund winter maintenance activities. Effective upon G&C approval through June 30, 2019. 100% Highway Funds. Contingent upon Fiscal Committee approval on March 8, 2019.

#27 Authorize the Bureau of Bridge Design to enter into an agreement with Jacobs Engineering Group Inc., Bedford, NH, for the preliminary design, public involvement process and associated environmental & cultural services for rehabilitation or replacement of the existing bridge carrying NH Route 12 over NH Railroad (abandoned) in the Town of Troy, for a total amount not to exceed $220,314.45. Effective upon G&C approval through June 30, 2021. 100% Federal Funds.

#28 Authorize the Bureau of Bridge Design to enter into an Interstate Bridge Agreement with the State of Vermont to allow the Department to bill Vermont for costs associated with bridge preservation work on the Ledyard Bridge (NH Br. No. 026/059) carrying NH Route 10A (Wheelock Street) over the Connecticut River between Hanover, NH and Norwich, VT. Effective upon G&C approval, and continuing through engineering design, construction, and final acceptance of the completed project, in accordance with the Department’s 10-year Transportation Improvement Plan. Project costs will be shared 69% NH, 31% VT.

#29 Authorize the Bureau of Traffic to enter into a contract with NH Logos LLC, Concord, NH, for the management of the Department’s Specific Service (logo) sign program. Effective April 1, 2019 through March 30, 2029, with the option of up to two, five-year renewals. No State Funding.

#30 Authorize the Bureau of Planning & Community Assistance to amend an existing contract with the Rockingham Planning Commission, Exeter, NH (originally approved by G&C on 6-21-17, item #64), to undertake certain transportation related planning activities, by increasing the amount by $23,818.50 from $1,104,154.20 to $1,127,972.70. Effective upon G&C approval through June 30, 2019. 100% Federal Funds.

#31 Authorize the Bureau of Transportation Systems Management and Operations to enter into an agreement with Vanasse Hangen Brustlin Inc., Bedford, NH, for on-call engineering and support services for Intelligent Transportation Systems initiatives statewide, for an amount not to exceed $750,000. Effective upon G&C approval through March 31, 2022.

#32 Authorize the Bureau of Bridge Design to enter into a Cooperative Agreement with the State of Maine to allow the Department to reimburse Maine for its estimated share of $28,714,157 for costs associated with bridge preservation construction on the Piscataqua River Bridge (NH Br. Nos. 257/127 and 258/128) carrying I-95 over the Piscataqua River between Portsmouth, NH, and Kittery, ME. Effective upon G&C approval through December 31, 2022, and continuing through engineering design, construction, inspection, and final acceptance of the completed project, in accordance with the Department’s 10 Year Transportation Improvement Plan. 100% Turnpike Funds.

#33 Authorize the Bureau of Construction to enter into a contract with SPS New England Inc., Salisbury, MA, for the design-build replacement of the Bunker Creek Bridge over US Route 4 in Durham, on the basis of best value, lump sum bid of $7,100,000. (2)Further authorize a contingency in the amount of $355,000 for payment of latent conditions which may appear during the construction of the project. (3)Further authorize $70,000 for stipend payments to the unsuccessful bidders, Severino Trucking Company Inc., and CPM Contractors Inc., ($35,000 each). Effective upon G&C approval through October 2, 2020. 100% Federal Funds.

#34 Authorize the Bureau of Aeronautics to amend a grant to provide additional funding to the Nashua Airport Authority, NH (originally approved by G&C on 12-7-16, item #25), to conduct airport pavement maintenance, rehabilitation and marking of Taxiway H and associated public-access taxilanes and aprons (south end of the airport) at Boire Field, Nashua, NH, by increasing the total Federal and State shares from $1,562,750 to $1,632,352.23, which equates to an increase of $69,602.23 or 95% of the grant amendment amount. Effective upon G&C approval through April 30, 2019. 95% Federal, 5% General Funds.

#35 Authorize the Bureau of Bridge Design to enter into an agreement with Jacobs Engineering Group Inc., Bedford, NH, for the preliminary design, public involvement process and associated environmental & cultural services for rehabilitation or replacement of the existing bridge carrying NH Route 12 over South Branch Ashuelot River in the Town of Troy, for an amount not to exceed $227,878.59. Effective upon G&C approval through June 30, 2021. 100% Other Funds (SB 367 Funds).

#35A Authorize the Bureau of Bridge Design to enter into an Interstate Bridge Agreement with the State of Vermont to allow the Department to bill Vermont for costs associated with bridge preservation work on the bridge (NH Br. No. 058/043) carrying Church Street over the Connecticut River and New England Central Railroad between Walpole, NH and Rockingham, VT. Effective upon G&C approval and continuing through engineering design, construction, inspection, and final acceptance of the completed project, in accordance with the Department’s 10-Year Transportation Improvement Plan under Statewide BRDG-HIB-M&P Program. Funds 74% NH, 26% VT.

COMMUNITY DEVELOPMENT FINANCE AUTHORITY
#36 Authorize, under the Community Development Block Grant program, to award a grant to the Town of Wolfeboro, NH, in the amount of $12,000 to support a feasibility study for the property situated at 23 Bay Street. Effective March 13, 2019 through June 30, 2020. 100% Federal Funds.

#39 Authorize to execute a sole source amendment to a previously competitively solicited contract with Dubois & King Inc., South Burlington, VT (originally approved by G&C on 6-10-15, item #43), for Professional Mechanical, Electrical and Plumbing Engineering Services, by increasing the amount by $150,000 from $250,000 to an amount not to exceed $400,000. #48 Authorize to enter into a contract with Bion C.
