Authorize the Division of Motor Vehicles to continue participation in a no cost memorandum of agreement with the US Department of Homeland Security, US Citizenship and Immigration Services, Washington, DC (originally approved by G&C on 4-3-13, item #74), for the purpose of verifying citizenship and immigration status information of non-citizen and naturalized or derived US citizen applicants applying for a NH driver license or non-driver identification card. Effective upon G&C approval through June 30, 2021. 100% Highway Funds.

DEPARTMENT OF ADMINISTRATIVE SERVICES
#B. Authorize to enter into an amendment with Bank of America, N.A., Charlotte, NC (originally approved by G&C On 7-10-13, item #16), for procurement card services, to add a no cost new enhancement that allows the use of Bank of America’s Receipt Imaging Service, and to extend the existing for an additional term of two years. #6 Authorize to enter into amendments with the vendors as detailed in letter dated March 14, 2016 (originally approved by G&C on 6-18-14, item #79), to exercise the renewal option in their existing agreements for the facilitation of the Family Assessment and Inclusive Reunification Program, by increasing the price by $549,928 from $545,928 to an amount not to exceed $1,095,856, and by extending the completion date from June 30, 2016 to June 30, 2018. 100% Federal Funds.

Division of Public Health Services
#7 Authorize to amend an existing agreement with the County of Cheshire (originally approved by G&C on 6-19-13, item #103), to develop a Climate and Health Adaptation Plan to assess and reduce the public health effects from severe weather events in the Greater Monadnock area, by increasing the total price by $40,000 from $12,696,418 to $12,736,418 Effective upon G&C approval through June 30, 2017. 100% Federal Funds.

DEPARTMENT OF TRANSPORTATION
#8 Authorize the Bureau of Right of Way’s request to accept the resignation of Richard R. Lemieux, Concord, NH, as a Highway Layout Commissioner for the projects: Barnstead 14121E, Franklin 13928A, and Pembroke 14477A. (2)Further, please note the passing of Thomas F. Tatarczach, Durham, NH who leaves the following projects vacant: Portsmouth 13455, Lee 15692, Farmington 16146, Farmington 16212, and Barrington 16178.

#9 Authorize the Bureau of Right of Way’s request to pay property owners $184,000 as documented in the Contemplated Awards List for amounts greater than $5,000 for the period extending from March 3, 2016 through March 31, 2016. Effective upon G&C approval.

#10 Authorize the Bureau of Planning and Community Assistance to enter into a sole source project agreement with the University of NH, Civil Engineering Department, Durham, NH, to administer the Local Technical Assistance Program for a fee not to exceed $787,502.87. Effective upon G&C approval through December 31, 2017. 100% Federal Funds.

#11 Authorize the Bureau of Highway Design to enter into an agreement with the firm of HNTB Corporation, Westbrook, ME, for final design engineering services to widen and retrofit the existing Bedford toll plaza facility on the F.E. Everett Turnpike to implement Open Road Tolling, in an amount not to exceed $737,399.35. Effective upon G&C approval through December 31, 2017. 100% Turnpike Funds.

#12 Authorize the Bureau of Construction to enter into a contract with Alvin J. Coleman & Son Inc., Conway, NH, for reconstructing and paving a ½ mile of gravel roadway on Intervale Road, beginning at Old Still Rd and proceeding east to just west of Battis Crossing Road, on the basis of a low bid of $398,497. (2)Further authorize a contingency in the amount of $39,849.70 for payment of latent conditions which may appear during the construction of the project. Effective upon G&C approval through August 26, 2016. 100% Betterment Funds.

#13 Authorize the Bureau of Construction to enter into a contract with Pike Industries Inc., Belmont, NH, for resurfacing of approximately 102 miles in District II in Grafton, Merrimack and Sullivan Counties on the basis of a single bid of $5,921,800. Effective upon G&C approval through September 30, 2016. 42.2% Betterment, 57.8% Agency Income.

#14 Authorize the Bureau of Construction to enter into a contract with Pike Industries Inc., Belmont, NH, for resurfacing of approximately 108 miles in District III in Belknap, Carroll, Grafton and Merrimack Counties on the basis of a single bid of $6,208,890. Effective upon G&C approval through September 30, 2016. 54% Betterment, 46% Agency Income.

#15 Authorize the Bureau of Construction to enter into a contract with Pike Industries Inc., Belmont, NH, for resurfacing of approximately 10.1 miles of mainline and ramps on the Spaulding Turnpike in the Town of Milton, on the basis of a low bid of $3,891,872.81. Effective upon G&C approval through September 9, 2016. 100% Turnpike Funds.

#16 Authorize the Bureau of Construction to enter into a contract with Alvin J. Coleman & Son Inc., Conway, NH, for the construction of a right turn lane on NH 104 approaching Meredith Center Road and a widened bypass shoulder on NH 104 opposite Chase Road, on the basis of a low bid of $886,786. (2)Further authorize a contingency in the amount of $62,075.02 for payment of latent conditions, which may appear during the construction of the project. Effective upon G&C approval through June 16, 2017. 100% Federal Funds.

#17 Authorize the Bureau of Construction to enter into a contract with Continental Paving Inc., Londonderry, NH, for resurfacing of approximately 58 miles in District V in Hillsborough, Merrimack and Rockingham Counties, on the basis of a low bid of $4,349,887.50. Effective upon G&C approval through September 30, 2016. 57% Betterment, 43% Agency Income.

#18 Authorize the Bureau of Construction to enter into a contract with Continental Paving Inc., Londonderry, NH, for resurfacing of approximately 70 miles in District VI in Merrimack, Rockingham and Strafford Counties, on the basis of a low bid of $4,483,310. Effective upon G&C approval through September 30, 2016. 48% Betterment, 52% Agency Income.

#19 Authorize the Bureau of Construction to enter into a contract with Pike Industries Inc., Belmont, NH, for resurfacing of approximately 79 miles in District I in Coos and Grafton Counties, on the basis of a single bid of $5,944,487.50. Effective upon G&C approval through September 30, 2016. 45.8% Betterment, 54.2% Agency Income.

#20 Authorize the Bureau of Construction to enter into a contract with Continental Paving Inc., Londonderry, NH, for resurfacing of approximately 5.0 miles of mainline and ramps on the F.E. Everett Turnpike in the City of Nashua, on the basis of a low bid of $3,168,828.61. Effective upon G&C approval through September 9, 2016. 100% Turnpike Funds.

DEPARTMENT OF INFORMATION TECHNOLOGY
#21 Authorize, on behalf of the Department of Health and Human Services, to exercise a contract amendment with Computer Associates Inc., Islandia, NY (originally approved by G&C on 6-4-08, item #4), to upgrade the speed of the mainframe software which will run an updated mainframe serving DHHS, by increasing the amount by $31,395 from $5,254,312 to $5,285,707. Effective upon G&C approval through June 30, 2016. 60% General, 40% Federal Funds.

#22 Authorize to enter into a sole source contract amendment to an existing contract with RTM Designs, Hendersonville, NC (originally approved by G&C on 7-15-09, item #6), for hosting, maintenance, and support for the Refer7 software application, by increasing the amount by $107,163.92 from $230,977 to $338,140.92, and by extending the end date of July 31, 2016 to June 30, 2020. 39% Federal, 61% General Funds.

THE ADJUTANT GENERAL’S DEPARTMENT
#23 Authorize to enter into a contract agreement with Securitas Security Services USA Inc., Manchester, NH, for providing armed security services at the Pease Air National Guard Base in Newington, NH, in the amount of $1,977,400. (2)Further authorize a general contingency in the amount of $39,948, bringing the total to $2,037,348. Effective May 1, 2016 through April 30, 2018. 100% Federal Funds.

DEPARTMENT OF CORRECTIONS
#24 Authorize to exercise a contract renewal option and amendment with Sentinel Offender Services LLC, Irvine, CA (originally approved by G&C 8-5-14, item #45), for the provision of Offender Electronic Monitoring Services for the NH Department of Corrections, by increasing the amount by $26,000 from $26,000 to $52,000. Effective July 1, 2016 through June 30, 2018. 90% Other, 10% General Funds.

#25 Authorize to exercise a contract renewal option and amendment with Johnstone Enterprises LTD d/b/a Granite State Shuttle Services, Dover, NH (originally approved by G&C on 9-3-14, item #60), for the provision of In-State Courier Services for the Northern NH Correctional Facility. Effective July 1, 2016 through June 30, 2018. 100% General Funds.

NEW HAMPSHIRE LIQUOR COMMISSION
#26 Authorize to grant a perpetual utility easement on State land to Unitil Energy Systems Inc., and Northern New England Telephone Operations LLC d/b/a FairPoint Communications – NNE), as tenants in common, for the construction, installation, operations, maintenance, and replacement of transmission and distribution lines and associated service poles and other supporting facilities and apparatus as needed to provide electrical and telecommunications utility service to the NH Liquor Commission Headquarters building in Concord, NH.

PUBLIC UTILITIES COMMISSION
#27 Authorize to award grant funds in the amount of $325,000 to Pemi-Baker Cooperative School District, to install a biomass boiler at Plymouth Regional High School in Plymouth, NH. Effective upon G&C approval through December 31, 2016. 100% Renewable Energy Fund, a non-lapsing special fund continually appropriated to the Commission.

#28 Authorize to award grant funds in the amount of $200,000 to Ever Better Hydro Power LLC, to recover, reactivate, and operate the retired Pittsfield Hydroelectric Project. Effective upon G&C approval thorugh June 30, 2017. 100% Renewable Energy Fund, a non-lapsing special fund continually appropriated to the Commission.

#29 Authorize to award grant funds in the amount of $200,000 to the University of NH, to install a steam turbine electric generator in Rudman Hall. Effective upon G&C approval through June 30, 2017. 100% Renewable Energy Fund, a non-lapsing special fund continually appropriated to the Commission.

#30 Authorize to award grant funds in the amount of $300,000 to Froling LLC, to install a dry wood chip fired biomass and a continuous feed wood chip drying facility located at its site in Peterborough, NH. Effective upon G&C approval through June 30, 2017. 100% Renewable Energy Fund, a non-lapsing special fund continually appropriated to the Commission.

NEW HAMPSHIRE FISH AND GAME DEPARTMENT
#31 Authorize to enter into a sole source contract with Prock Marine Company, Rockland, ME, for the installation of Piles at Newfound Lake, Bristol, NH, in the amount of $89,882. Effective upon G&C approval through June 30, 2016. 75% Federal, 25% Statewide Public Boat Access Funds.

#32 Authorize to enter into a Cooperative Project Agreement with University of NH to carry out endangered seabird conservation, for a total of $85,053. Effective upon G&C approval through April 22, 2017. 65% Federal, 35% Conservation License Plate Funds.

#33 Authorize to enter into a transfer agreement with Northeast Fishery Sector XI for the transfer of Annual Catch Entitlement of groundfish from federal fisheries permits residing in the NH State Permit Bank. Effective upon G&C approval through April 30, 2017.

#34 Authorize to enter into a sole source no cost Cooperative Project agreement with the University of NH Cooperative Extension 4-H Camps, Durham, NH, for the purpose of operating the Barry Conservation Camp located at the Berlin Fish Hatchery. Effective upon G&C approval through June 30, 2021.

#35 Authorize to enter into a contract with Holden Engineers & Surveyors Inc., Bedford, NH, to conduct complete boundary survey and subdivision of the Johnson Family farm property in Grafton, NH, in the amount of $19,876. Effective upon G&C approval through June 15, 2016. 25% Wildlife habitat account and 75% Federal Funds.

DEPARTMENT OF ENVIRONMENTAL SERVICES
#36 Authorize to accept and expend other funds in the amount of $11,232 from the Department of Safety, Division of Homeland Security and Emergency Management. Effective upon G&C approval through September 30, 2016. 100% Other (Interagency Federal Pass-Thru) Funds.

#37 Authorize to award an Asset Management grant to the Town of Lincoln, NH, in the amount of $15,000 to improve public water system management. Effective upon G&C approval through May 31, 2017. 100% Federal Funds.

#38 Authorize to enter into a grant agreement with the Town of Moultonborough, NH, in the amount of $59,000 to fund exotic aquatic plant control activities. Effective upon G&C approval through December 31, 2016. 100% Lake Restoration Funds.

#39 Authorize to amend an agreement with Watershed Assessment Associates LLC, Schenectady, NY (originally approved by G&C on 6-10-15, item #77), for the purpose of processing aquatic benthic samples and taxonomic identification for macroinvertebrates, by increasing the amount by $7,818 from $134,215 to $142,033. Effective upon G&C approval through May 21, 2020. 100% Federal Funds.

#40 Authorize to award a Local Source Water Protection grant to the Green Mountain Conservation Group, Effingham, NH, in the amount of $19,387 to complete a project to protect public drinking water systems. Effective upon G&C approval through May 31, 2017. 100% Federal Funds.

#41 Authorize to award a Local Source Water Protection grant to Lower Bartlett Water Precinct in the amount of $8,908 to complete a project to protect public drinking water systems. Effective upon G&C approval through May 31, 2017. 100% Federal Funds.

#42 Authorize to award a Local Source Water Protection grant to the Rockingham Planning Commission, Exeter, NH, in the amount of $9,500 to complete a project to protect public drinking water systems. Effective upon G&C approval through May 31, 2017. 100% Federal Funds.

#43 Authorize to award a Local Source Water Protection grant to the Lake Winnipesaukee Association, Meredith NH, in the amount of $16,770 to complete a project to protect public drinking water systems. Effective upon G&C approval through May 31, 2017. 100% Federal Funds.

#44 Authorize to enter into an agreement with the University of NH, Durham, NH, to complete the Great Bay Watershed Improvements to Reduce Nitrogen on the UNH Campus project, in the amount of $65,000. Effective upon G&C approval through June 30, 2017. 100% Federal Funds.

#45 Authorize a Clean Water State Revolving Fund loan agreement with the Town of Allenstown in the amount not to exceed $30,000 to finance an asset management plan for their wastewater collection and treatment system. Effective upon G&C approval. 100% CWSRF Repayment Funds.

#46 Authorize a Clean Water State Revolving Fund loan agreement with the City of Portsmouth, in an amount not to exceed $75,000,000 to finance with Peirce Island Wastewater Treatment Facility Upgrade project. Effective upon G&C approval. 100% CWSRF Repayment Funds.

#47 Authorize a loan agreement with the White Lake Estates Homeowners Association Inc., Tamworth, NH, in the amount not to exceed $292,750 to finance water system improvements. Effective upon G&C approval. 100% Drinking Water State Revolving Loan Fund Repayment Funds.

#48 Authorize NH Fish & Game Department’s request for a five year permit time extension to perform work on Hot Hole Pond in Concord, NH.

DEPARTMENT OF EDUCATION
#49 Authorize to enter into a sole source contract with the National Center for Competency Based Learning, Durham, NH, to administer the workplace competency compounds of the E3 Teen Fathering Program, in an amount not to exceed $220,000. Effective upon G&C approval through June 30, 2017, with the option to renew for up to three additional one-year terms. 100% Federal Funds.

DEPARTMENT OF SAFETY
#50 Authorize to enter into a grant agreement with the Department of Health and Human Services, in the amount of $69,000 for the State Homeland Security Program portion of the 2015 Homeland Security Grant Program. Effective upon G&C approval through August 31, 2018. 100% Federal Funds.

DEPARTMENT OF JUSTICE
#51 Authorize to conduct seven Municipal Trustee Seminars in an amount not to exceed $4,000; three seminars at the Town and Country Motor Inn, Shelburne, NH and four seminars at the Holiday Inn, Concord, NH. Effective upon G&C approval through June 30, 2016. 100% Agency Income.

#55 Authorize the Division of Personnel’s request for waiver of classification decisions. Effective upon G&C approval.

#56 Authorize to retroactively enter into a third amendment to the current contract with Banc of America Merchant Services LLC (“BAMS”), and Bank of America N.A., Jamaica Plain, MA (originally approved by G&C on 9-18-13, item #27E), for merchant card processing services, by extending the date of Conversion Assistance for conversion from the First Data processing platform to the BAMS processing platform, by extending the end date from March 15, 2016 to October 28, 2016. No Additional Costs.

#57 Authorize the Division of Public Works Design and Construction to enter into a contract with GMI Asphalt LLC, Belmont, NH, for the Reclaim and Repave the Flume Parking Lot – Franconia Notch Park Daniel Webster Highway, Lincoln, NH, for a total price not to exceed $512,834. (2)Further authorize the amount of $4,000 for payment to the Department of Administrative Services, Division of Public Works Design and Construction, Capital Clerk for oversight and engineering services provided, bringing the total to $516,834. Effective upon G&C approval through September 30, 2016. 100% General – Capital Funds.

#58 Authorize the Bureau of Public Works Design and Construction to enter into a contract with Charters Brothers Construction, Danville, NH, for the Bureau of Traffic Building A, Boiler Replacement, Concord, NH, for a total price not to exceed $249,400. Effective upon G&C approval through August 15, 2016. 84% Highway, 16% General Funds. (2)Further authorize the amount of $5,027 for payment to the Department of Administrative Services, Division of Public Works Design and Construction, for engineering services provided, bringing the total to $254,427. 100% Highway Funds.
