Authorize the Bureau of Highway Design to amend an agreement with McFarland-Johnson Inc., Concord, NH and Binghamton, NY (originally approved by G&C on 11-20-13, item #123), to prepare the second phase of the preliminary engineering design for the section of Interstate 93 passing through Bow and Concord from I-89 through I-393, by extending the completion date from December 31, 2015 to June 30, 2017. No Additional Funding.

DEPARTMENT OF TRANSPORTATION
#B. Authorize the Bureau of Highway Design to amend an agreement with Jacobs Engineering Group Inc., Bedford, NH (originally approved by G&C on 8-10-11, item #135), for the final design for the reconstruction and widening of a 2.7 mile section of NH 12 in the Towns of Walpole and Charlestown, by extending the completion date from December 31, 2015 to December 31, 2016. #C. BUSINESS FINANCE AUTHORITY
#5 Authorize to hold a Public Hearing to approve an award of a State Guarantee under RSA 162-A:13-a with respect to a loan from The Provident Bank to Palmer & Sicard, Inc. located in Exeter, New Hampshire. Action will authorize a Resolution under RSA 162-A:18 to determine the financing will serve a public use, provide public benefit and is within the policy and authority of the Act.

#6 Authorize to finalize the determination made by the Commissioner that on September 21, 2015 Andrew Holmes, a Youth Counselor III at the Sununu Youth Services Center sustained an injury in the line of duty due to a hostile or overt act, or an act caused by another during the performance of duties which are considered dangerous in nature. Effective upon G&C approval.

#7 Authorize to make a retroactive one-time payment from SFY 2016 funds to Christine Keane Registered Nurse II, NH Hospital in the amount of $2,441.78 to compensate for wages that were not paid for SFY 2015. 27% General, 43% Other, 30% Federal Funds.

Office of Medicaid Business and Policy
#9 Authorize to enter into an amendment to a sole source agreement with the University of NH, Institute for Health Policy and Practice, Durham, NH (originally approved by G&C on 6-5-13, item #86), to develop additional quality and calculation measures for the Adult Medicaid Quality project, by increasing the price by $49,915 from $7,022,452 to $7,072,367. Effective upon G&C approval through the original end date of June 30, 2017. 100% Federal Funds.

Public Health Services
#10 Authorize the Bureau of Population Health and Community Services, Maternal & Child Health Section to enter into a retroactive memorandum of agreement with the Department of Justice, Concord, NH, to continue the development of the Sudden Unexpected Infant Death Case Registry, in an amount not to exceed $62,391. Effective September 1, 2015 through August 31, 2018. 100% Federal Funds.

#11 Authorize to enter into a sole source agreement with the AXYS Analytical Services Ltd., Sidney, Canada, to provide analytical laboratory testing services in an amount not to exceed $146,547. Effective upon G&C approval through June 30, 2017. 100% General Funds.

#12 Authorize to add Frisbie Memorial Hospital, Rochester, NH to a list of licensed medical providers, with the ability to expand to include additional licensed medical providers to provide necessary outpatient visits, labs, diagnostic tests, and outpatient procedures for clients enrolled in the NH Ryan White CARE Program. No Maximum client or services volume is guaranteed. Accordingly, the price among all agreements is $100,000. Effective upon G&C approval through June 30, 2016. 100% Other Funds.

#13 Authorize to enter into an agreement with JSI Research & Training Institute Inc., d/b/a Community Health Institute, Bow, NH, to provide consultant and planning services to improve NH Ebola preparedness and emerging infectious disease readiness in hospitals and the healthcare system statewide, in an amount not to exceed $110,000. Effective upon G&C approval through September 30, 2016. 100% Federal Funds.

#14 Authorize the Bureau of Population Health and Community Services, Maternal and Child Health Section, Home Visiting Program, to enter into a retroactive sole source amendment with Social Solutions Global Inc., Baltimore, MD (originally approved by G&C on 9-19-12, item #57), to provide the State with a hosted, secure, web-based home visiting software system which includes support and maintenance services, by increasing the price by $76,847.75 from $257,277.25 to $334,125 and extending the contract completion date from September 30, 2015 to March 31, 2016. 100% Federal Funds.

DEPARTMENT OF TRANSPORTATION
#15 Authorize the Bureau of Highway Maintenance to retroactively pay prior year invoices to Unitil Energy Sys, Boston, MA, in the amount of $3,781.73. Effective upon G&C approval. 83% Highway, 17% Other Funds.

#16 Authorize the Bureau of Materials & Research to execute a sole source Cooperative Project Agreement with the UNH Sponsored Programs Administration, Durham, NH, for a cooperative investigation to advance the science and engineering of tidal energy conversion, sensor network development and deployment for structural health monitoring, intelligent transportation system, innovative monitoring and community engagement, for a total fee not to exceed $455,000. Effective upon G&C approval through May 31, 2019. 100% Federal Funds.

#17 Authorize the Bureau of Mechanical Services to amend the Equipment Acquisition Plan (originally approved by G&C on 8-26-15, item #27), by adding additional equipment as result of an additional $600,000 included in the appropriated budget. Effective upon G&C approval through June 30, 2017.

#18 Authorize the Bureau of Environment to pay the Department of Environmental Services Aquatic Resource Mitigation Fund $224,948.38 as documented in E-mail correspondence, approval letters, and ARM fund calculators from DES. Effective upon G&C approval through June 30, 2016. 100% Federal Funds.

#19 Authorize the Bureau of Turnpikes to exercise a contract renewal option with Travelers Marketing LLC, Wellesley, MA (originally approved by G&C on 11-14-12, item #164), for the Motor Safety Patrol in an amount of $254,490, for a fee to be paid to the Bureau of Turnpikes beginning November 30, 2015 to November 30, 2017, increasing the price from $263,790 to $518,280.

#20 Authorize the Bureau of Aeronautics to award a grant to the Laconia Airport Authority, to perform airport pavement maintenance and paint marking and to prepare a Storm Water Pollution Prevention Plan at the Laconia Municipal Airport, in the amount of $155,282.78. Effective upon G&C approval through November 30, 2019. 90% Federal, 5% General, 5% Local Funds.

#21 Authorize the Bureau of Aeronautics to amend a grant with the Town of Whitefield, NH (originally approved by G&C on 11-17-10, item #207), to design, reconstruct and expand the terminal aircraft-parking apron at the Mt. Washington Regional Airport, by increasing the funding by $14,043.68 from $775,904 to $789,947.68. Effective upon G&C approval. 95% Federal, 5% General Funds.

#22 Authorize the Bureau of Construction to enter into a contract with Winterset Inc., Belmont, NH, for bridge rehabilitation on the bridge carrying US 2 over Israel River in the Town of Jefferson, on the basis of a low bid of $1,173,473.50. (2)Further authorize a contingency in the amount of $117,347.35 for payment of latent conditions, which may appear during the construction of the project. Effective upon G&C approval through November 18, 2016. 100% Federal Funds.

#23 Authorize the Bureau of Construction to enter into a contract with Miller Construction Inc., Windsor, VT, for bridge rehabilitation work on US 4 over the Mascoma River in the Town of Canaan, on the basis of a low bid of $1,022,151.50. (2)Further authorize a contingency in the amount of $102,215.15 for payment of latent conditions, which may appear during the construction of the project. Effective upon G&C approval through October 28, 2016. 100% Federal Funds.

#32 Authorize to award a sole source grant to the University of NH, Sponsored Programs Administration, Durham, NH, in the amount of $134,301 to assess the costs and benefits of a family leave insurance program in NH. Effective upon G&C approval through September 30, 2016. 100% Federal Funds.

PUBLIC UTILITIES COMMISSION
#33 Authorize to continue membership and participation in the New England Conference of Public Utilities Commissioners Inc., in the amount of $24,000. Effective retroactive to October 1, 2015 through September 30, 2016. 100% Other Funds (Utilities Assessment).

#34 Authorize to award grant funds in the amount of $1,200,000 to Public Service of NH d/b/a Eversource Energy, to extend fuel-neutral program services to both retail businesses and large energy users that commit to cost-effective energy efficiency improvements. Effective upon G&C approval through December 31, 2018. 100% Energy Efficiency Fund, a non-lapsing special fund continually appropriated to the Commission.

OFFICE OF THE CONSUMER ADVOCATE
#35 Authorize to enter into a contract with Synapse Energy Economics Inc., to monitor regional market developments and assist in participation in the regulation, governance and operation of the New England regional electric power markets and transmission grid, in the amount of $40,000. Effective upon G&C approval through June 30, 2017. 100% Public Utility Assessments.

DEPARTMENT OF RESOURCES AND ECONOMIC DEVELOPMENT
#36 Authorize the Division of Parks and Recreation, Cannon Mountain, to enter into a sole source addition of Schedule #8 to the previously approved Master Lease Equipment agreement with International Financial Services Corporation, Libertyville, IL (originally approved by G&C on 12-9-09), in the amount of $393,216 for the 41-month lease of one new Prinoth Bison TW. Effective December 1, 2015 through May 1, 2019. 100% Ski Area Funds.

#37 Authorize the Division of Parks and Recreation to enter into a sole source contract with Warrenstreet Architects Inc., Concord, NH, for architectural and engineering services for improvements to toilet and shower facilities in various state parks, in the amount of $14,000. Effective upon G&C approval through June 30, 2017. 100% Capital Funds.

#38 Authorize the Division of Forests and Lands to enter into a retroactive lease agreement with NH#1 Rural Cellular Inc., Chicago, IL, for equipment and antenna space in designated areas at Mount Sunapee State Park. (2)Further authorize to accept annual rental payments of $33,010.80 for year one, $34,001.12 for year two, $35,021.16 for year three, $36,071.79 for year four, and $37,153.95 for year five. Effective July 1, 2015 through June 30, 2020, with the option to renew for three additional 5-year periods.

#39 Authorize the Division of Forests and Lands to enter into a contract with J.P. Macdonald Services, LLC, Georgetown, MA, for Phase 2 of Ground Accessible State Fire Tower Upgrades, in the amount of $84,420. (2)Further authorize a contingency in the amount of $13,000 to cover any unforeseen costs necessary. Effective upon G&C approval through August 1, 2016. 100% Capital Funds.

NEW HAMPSHIRE FISH AND GAME DEPARTMENT
#40 Authorize to accept the transfer of a conservation easement on 31 acres in Loudon from ABJEH Realty LLC. Effective upon G&C approval.

#41 Authorize to enter into a sole source contract with the Wildlife Management Institute to provide a scope of services that includes research and management of Cottontail, for the amount not to exceed $425,300. Effective upon G&C approval through June 30, 2017. 94% Federal, 6% Conservation License Plate Funds.

DEPARTMENT OF ENVIRONMENTAL SERVICES
#42 Authorize to amend a Grant Agreement with the City of Keene, NH, (originally approved by G&C on 3-26-14, item #52), to complete the project involving the restoration of approximately one acre of emergent/scrub-shrub wetlands previously disturbed at Woodlawn Cemetery, by extending the end date to June 30, 2017 from December 31, 2015. No Additional Funding. 100% ARM Funds.

#43 Authorize to award an Aquatic Resource Mitigation Fund grant to The Monadnock Conservancy, Keene, NH, in the amount of $140,000 to acquire a conservation easement on two parcels of land totaling 552 acres in Keene, Swanzey and Chesterfield. Effective upon G&C approval through June 1, 2016. 100% Aquatic Resource Mitigation Funds.

#44 Authorize to enter into retroactive grant agreements with the entities as detailed in letter dated September 30, 2015, totaling $115,404 to fund Household Hazardous Waste collections. Effective July 1, 2015 through June 30, 2016. 100% Hazardous Waste Cleanup Funds.

#45 Authorize to retroactively amend a contract with Applied Geographics Inc., Boston, MA (originally approved by G&C on 8-15-14, item #68), in order to complete the development of the NH Environmental Indicators Dashboard Design and Content, by extending the completion date to December 31, 2015 from September 30, 2015. No Additional Funding. 100% Federal Funds.

#46 Authorize the NH Department of Transportation’s request to perform work on an unnamed brook and the Atlantic Ocean in Rye.

#49A Authorize to enter into a sole source contract with the Regional Greenhouse Gas Initiative Inc., New York, NY, to provide technical and administrative services for continuation of the NH Carbon Dioxide Budget Trading Program, in an amount not to exceed $375,000. (2)Further authorize to make annual advance payments upon invoice by RGGI Inc. Effective upon G&C approval through December 31, 2018. 100% CO2 Trading Funds.

DEPARTMENT OF EDUCATION
#50 Authorize the Bureau of Assessment and Accountability to enter into a sole source agreement with the College Board, New York, NY, to administer the SAT assessment in grade 11 to ensure a continuation of assessment services to meet federal and state law, in an amount not to exceed $613,272.45. Effective upon G&C approval through June 30, 2016, with the option to renewal for up to one additional fiscal year. 27% State, 73% Federal Funds.

#51 Authorize the Division of Career Technology and Adult Learning to retroactively grant funds to NH Coalition for Occupational Safety and Health, Hooksett, NH, to provide English Language/Civics Education classes to workers who are learning English as another language and to students in adult basic education classes throughout the state, in an amount not to exceed $28,454. Effective September 1, 2015 through June 30, 2016. 35% Federal, 65% General Funds.

DEPARTMENT OF JUSTICE
#52 Authorize to enter into a subgrant with the NH Coalition Against Domestic and Sexual Violence, in the amount of $291,682 from the US Department of Justice, Office on Violence Against Women, Sexual Assault Support Program, for the purpose of providing services for victims of sexual assault in NH. Effective upon G&C approval through July 31, 2017. 100% Federal Funds.

#53 Authorize the Office of Victim Witness to accept and expend additional grant funds of $51,340 from the NH Department of Justice for the purposes of providing NH’s colleges and universities with information and support to develop best practices for responding to sexual violence on college campuses, develop first responder trainings and promote cross training among crisis centers, law enforcement, colleges and universities. Effective upon G&C approval through June 30, 2017. 100% Agency Income (origin-federal funds).

#53A Authorize to grant an annual salary increment to Kathleen B. Carr, Director of Administration, from Salary Group GG Step 3 $86,652.44 (NH First, Step 4) to Salary Group GG, Step 4, $91,401.26 (NH First Step 5). Effective December 12, 2015, or upon G&C approval, whichever is the latter.
