#6           Authorize to hold a Public Hearing with respect to the financing of a commercial facility by the Business Finance Authority for The Baldwin Senior Living, for a project in Londonderry, New Hampshire.  Action will authorize a Resolution under RSA 162-I:9 entitled “A PLAN OF FINANCING FOR CERTAIN CAPITAL ASSETS FOR A CONTINUING CARE RETIREMENT COMMUNITY, INCLUDING THE ACQUISITION OF LAND AND THE CONSTRUCTION, EQUIPPING AND FURNISHING OF THE FACILITY AND ITS RELATED AMENITIES AND COMMON FACILITIES, BY THE BUSINESS FINANCE AUTHORITY FOR THE BALDWIN SENIOR LIVING IN THE TOWN OF LONDONDERRY”  to determine the financing will serve a public use, provide public benefit and is within the policy and authority of the Act.

#7           Authorize to enter into a new lease with ZJBV Properties LLC, Manchester, NH, for warehouse space at 70 Pembroke Road, Concord, NH, for the Emergency Services Unit and NH Hospital, in the amount of $1,036,051.20.  Effective upon G&C approval through May 31, 2025.  49% General, 51% Federal Funds.   

                              Office of the Commissioner 
#7A        Authorize the Division of Medicaid Services to retroactively amend existing agreements with AmeriHealth Caritas NH Inc., Philadelphia, PA; Boston Medical Center Health Plan Inc., Charlestown, MA; and Granite State Health Plan Inc., Bedford, NH, to provide health care services to eligible and enrolled Medicaid participants through NH’s Medicaid managed care program known as NH Medicaid Care Management.  The shared price remains unchanged, not to exceed $816,565,089, in SFY 2020, retroactive to September 1, 2019, with no change to the completion date of August 31, 2021.  After reconciliation of membership a reallocation of funds was done within the price limitation as detailed in letter dated April 24, 2020.  Funds for Granite Advantage Health Program are 93% Federal, 7% Other for calendar year 2019 and 90% Federal and 10% Other for calendar year 2020; funds for the Child Health Insurance Program are 77.6% Federal and 22.4% General; and funds for the standard Medicaid population funding under the Medicaid Care Management account are 51% Federal, 22.7% General, and 26.3% Other Funds.   

                              New Hampshire Hospital 
#8           Authorize to amend an existing contract with Achievement Therapy Services LLC, Laconia, NH (originally approved by G&C on 4-11-18, item #12), to provide physical therapy services to clients admitted to NH Hospital, by exercising a contract renewal option by increasing the price by $190,000 from $202,500 to $392,500, and by extending the completion date from June 30, 2020 to June 30, 2022.  34% General, 66% Other Funds.   

                              Division of Economic & Housing Stability 
#9           Authorize to amend an existing sole source contract with Waypoint, Manchester, NH (originally approved by G&C on 6-5-19, item #33), to continue providing a Rapid Re-Housing Program serving NH Runaway and Homeless Youth, through the Federal Continuum of Care Program, by exercising a contract renewal option, by increasing the price by $200,112 from $188,950 to $389,062, and by extending the completion date from May 31, 2020 to April 30, 2021.  100% Federal Funds.   

#10        Authorize to amend existing sole source contracts with one of the listed vendors: Community Action Program Belknap and Merrimack Counties Inc, Concord, NH (originally approved by G&C on 6-29-16, item #16), for the provision of Emergency Solutions Grant Services, by increasing the total price by $74,852 from $3,245,497 to $3,320,349.  Effective upon G&C approval through the original completion date of June 30, 2021.  100% Federal Funds.   

                              Division of Public Health Services 
#11        Authorize to amend an existing contract with Abacus Service Corporation, Southfield, MI (originally approved by G&C on 11-14-18, item #17), for information technology support for the Automated Hospital Emergency Department Data system, by increasing the price by $10,000 from $231,189 to $241,189, and by extending the completion date from August 31, 2020 to August 31, 2021.  100% Federal Funds.   

#12        Authorize to amend an existing sole source cooperative project agreement with the University of NH, Durham, NH (originally approved by G&C on 6-20-18, item #26), to continue providing epidemiological expertise, by increasing the price by $90,000 from $565,000 to $655,000.  Effective upon G&C approval the original completion date of June 30, 2021.  100% Federal Funds.   

#13        Authorize to amend an existing sole source contract with Bi-State Primary Care Association Inc., Bow, NH (originally approved by G&C on 7-11-18, item #11), for the recruitment of primary care healthcare, dental and behavioral health professionals to medically underserved areas of NH, by increasing the price by $554,000 from $654,000 to $1,208,000, and by extending the completion date from June 30, 2020 to June 30, 2022.  25% Federal, 75% General Funds.   

#14        Authorize to amend an existing sole source agreement with JPMA Inc., Wheat Ridge, CO (originally approved by G&C on 7-11-18, item #10), for the mobile application that assist Women, Infants and Children participants in utilizing WIC benefits, by increasing the price by $52,800 from $61,200 to $114,000, and by extending the completion date from June 30, 2020 to June 30, 2022.  (2)Further authorize advance payment in the amount of $26,400.  100% Federal Funds.   

#15        Authorize to amend an existing sole source contract with Jeff Murray’s Programming Shop Inc., New Orleans, LA (originally approved by G&C on 4-9-14, item #45), for support and maintenance on the NH Ryan White Programs CareWare system, by increasing the price by $72,000 from $77,280 to $149,280, adding four years to the contract, and extending the completion date from June 30, 2020 to June 30, 2024.  100% Other Funds (Pharmaceutical Rebates).   

#16        Authorize to enter into a contract with Lamprey Health Care Inc., Newmarket, NH, for implementation of evidence-based interventions focused on improving breast and cervical cancer screening rates, in the amount of $90,000.  Effective upon G&C approval through June 30, 2022, with the option to renew for up to two additional years.  100% Federal Funds.   

#17        Authorize to amend an existing sole source agreement with Abacus Services Corporation, Southfield, MI (originally approved by G&C on 6-24-15, item #53), to continue providing operations, maintenance and development services for the NH Web-based Interactive System for Direction and Outcome Measures, by increasing the price by $132,000 from $1,037,944 to $1,169,944 and by extending the completion date from June 30, 2020 to June 30, 2021.  33% Federal, 67% Other Funds.   

                              Division of Long Term Supports and Services 
#18        Authorize to award contracts with the vendors as detailed in letter dated April 30, 2020, to provide Aging and Disability Resource Center ServiceLink services in ten geographic areas of the state to improve access to information on the full range of long-term services and supports, in an amount not to exceed $6,443,554.02.  Effective July 1, 2020 or upon G&C approval, whichever is later, through June 30, 2022, with the option to renew for up to two additional years.  56.67% Federal, 43.33% General Funds.   

                              DEPARTMENT OF TRANSPORTATION 
#19        Authorize the Bureau of Right of Way’s request to pay property owners $1,230,000 as documented in the Contemplated Awards List for amounts greater than $5,000 for the period extending from April 15, 2020 through April 27, 2020.  Effective upon G&C approval. 

#19A     Authorize the Division of Project Development to transfer $15,400,000 between various accounts and classes.  Effective upon G&C approval through June 30, 2020.  Fiscal Committee approved.   

#19B      Authorize the Bureau of Right of Way to pay property owners $3,225,000 as documented in the Contemplated Awards List, for amounts greater than $5,000 for the period extending from March 20, 2020 through April 14, 2020.  Effective upon G&C approval.   

#20        Authorize the Bureau of Traffic to accept and expend revenue in the amount of $50,000 from the National Highway Traffic Safety Administration through NH Department of Safety, Office of Highway Safety, for the purpose of purchasing and installing signs along limited access highways reminding motorists of the statutory requirements to engage in driving without hand held electronic devices.  Effective upon G&C approval through June 30, 2021.  100% Agency Income.   

#21        Authorize the Bureau of Construction to enter into a contract with TMI Services Inc., Franklin, MA, for replacement of pavement markings along the Turnpike system, on the basis of a low bid of $1,010,152.35.  Effective upon G&C approval through August 27, 2021.  100% Turnpike Funds.   

#22        Authorize the Bureau of Highway Design to amend a contract with Vanasse Hangen Brustlin Inc., (originally approved by G&C on 2-1-17, item #26), for preliminary design of improvements to US 202 at its intersection with NH 124 and NH 137 in the Town of Jaffrey, by retroactively extending the completion date from April 30, 2020 to August 31, 2020.  No Additional Funding. 

                              OFFICE OF THE GOVERNOR 
#22A     Authorize to accept and expend funds in the amount of $30,000 from the Department of Health and Human Services, Division for Children, Youth and Families, to provide support services to DHHS to implement the Choose Love learning curriculum across programs connected to Children Youth and Families.  Effective upon G&C approval through June 30, 2021.  100% Transfer Funds.   

                              COMMUNITY DEVELOPMENT FINANCE AUTHORITY 
#23        Authorize, under the Community Development Block Grant Program, to award a grant to the Town of Salem, NH, in the amount of $245,000 on behalf of the Salem Housing Authority to complete building revitalization activities at the Millville Arms Apartments, Salem, NH.  Effective May 20, 2020 through December 31, 2021.  100% Federal Funds.   

#24        Authorize, under the Community Development Block Grant Program, to award a grant to the City of Claremont, NH, in the amount of $25,000 on behalf of Sugar River Cooperative Inc., to support its water and sewer infrastructure systems study at the property, Claremont, NH.  Effective May 20, 2020 through June 30, 2022.  100% Federal Funds.   

                              DEPARTMENT OF INFORMATION TECHNOLOGY 
#25        Authorize, on behalf of the Department of Health and Human Services, to enter into a sole source contract amendment with RTM Designs, Hendersonville, NC (originally approved by G&C on 7-15-09, item #6), to provide continued hosting, maintenance, and support for the Refer7 software application, by increasing the amount by $200,597 from $369,957.92 to $570,554.92, and by extending the completion date from June 30, 2020 to June 30, 2024.  58% General, 42% Federal Funds.   

#26        Authorize to amend an existing sole source contract agreement with The Center for Internet Security, Greenbush, NY (originally approved by G&C on 6-1-16, item #49), for the purpose of continued cyber security monitoring services, in the fixed firm amount of $28,752 increasing from $59,926 to $85,678, and by extending the completion date to June 30, 2022 from June 30, 2020.  100% Capital Funds.

#40        Authorize to accept and expend a Carl D. Perkins sub grant from the NH Department of Education, in the amount of $10,000 to provide funding for the Career and Technical Education programs and classes at the NHDOC facilities.  Effective upon G&C approval through June 30, 2021.  100% Federal Revenue received from another State Agency.   

#41        Authorize to enter into a contract agreement with Diplomatic Security LLC, Pembroke, NH, for the provision of In-State Courier services, in the amount of $200,720.  Effective July 1, 2020 through June 30, 2023, with the option to renew for one additional period of two years.  100% General Funds.   

                              NEW HAMPSHIRE FISH AND GAME DEPARTMENT 
#42        Authorize to transfer to the Department of Environmental Services, ownership of the Weeks Crossing Dam in Warren NH, along with all property and water rights associated with the dam.  Also, in accordance with RSA 482:51 III, authorize NHES’s acceptance of the above transfer.   

#43        Authorize to enter into a contract with Joe Brigham Inc., Pembroke, NH, to carry out aerial stocking and water sample collection (acid rain analysis) of remote, high altitude trout ponds, in the amount of $15,025.  Effective upon G&C approval through June 30, 2020.  75% Federal, 25% Fish and Game Funds.   

                              DEPARTMENT OF BUSINESS AND ECONOMIC AFFAIRS

#61        Authorize expenditures from the Drinking Water and Groundwater Trust Fund with the public schools as detailed in letter dated April 30, 2020.  Effective upon G&C approval.  100% Drinking Water Trust Fund.   

#62        Authorize the Bureau of Student Support to enter into an amendment to a contract with Creative Educational Consulting LLC, Hollis, NH (originally approved by G&C on 10-17-18, item #34), for training and support, by increasing the price by $25,000 from $160,000 to $185,000 and by extending the completion date from June 30, 2020 to September 30, 2020.  100% Federal Funds.

#63        Authorize the Division of State Police to enter into a contract with Northeast Collision Center LLC, New London, NH, for reconditioning and transport of vehicles to the White Farm before auction, in an amount not to exceed $66,496.  Effective upon G&C approval through June 30, 2023, with an option to renew for one 2-year period at the sole decision of the State.  51% General, 29.87% Highway, 15.03% Turnpike Funds.

#68        Authorize to enter into a subgrant award with the NH Coalition Against Domestic and Sexual Violence, in the amount of $107,000 from the Federal Victim of Crime Act Grant for the purpose of the customization, implementation and training for a new victim services database retroactively from April 8, 2020 through September 30, 2020.  100% Federal Funds.   

#69        Authorize to enter into subgrants with the organizations as detailed in letter dated April 30, 2020, in the total amount of $483,073 from the NH 2019 Statewide Drug Court Project Grant, to elevate the use of evidence-based practices and principles; build capacity in risk and needs assessment; ensure access to evidence-based services; expand the use of recovery support services; and facilitate successful recovery for all adult Drug Court participants.  Effective upon G&C approval through September 30, 2022.  100% Federal Funds.   

#70        Authorize consideration of whether the petition of Nicole Kasinskas (age 32) requesting a commuted sentence hearing for the offense of Attempted Second Degree Murder and Conspiracy to Commit First Degree Murder should be granted.  The petitioner is not eligible for an annulment.   

#71        Authorize to enter into subgrants with the organizations as detailed in letter dated May 4, 2020, in the total amount of $6,869,816.80 from the Federal Victim of Crime Act Grant for the purpose of providing direct services to victims of crime.  (2)Further authorize to enter into subgrants as the result of a competitive Request for Proposal process, with the organizations as detailed in letter dated May 4, 2020, in the total amount of $1,190,292.40 from the Federal Victim of Crime Act Grant for the purpose of providing direct services to victims of crime.  Effective July 1, 2020 through September 30, 2022.  100% Federal Funds.
