               #A.         Authorize to enter Allysa Voisine into an educational tuition agreement with Southern NH University – College for America, Manchester, NH, to participate in Modules in Business Management, including: Public Fiscal Management, Public Administration, and Policy Analysis and the Role of the Public, during the period of March 1, 2020 through August 31, 2020, and to pay said costs in an amount of $1,500.  20% Federal, 80% General Funds.

#2           MOP 150, I, B (2): Approval of Acceptances 

                                             NEW HAMPSHIRE VETERANS HOME 
               #A.         Authorize to accept the non-monetary donations totaling $26,580 received during the period of October 1, 2019 to December 31, 2019 as a result of contributions. 

                                             NEW HAMPSHIRE VETERANS HOME 
               #B.         Authorize to accept the list of monies totaling $402,643.30 received during the period of October 1, 2019 to December 31, 2019 as a result of fundraising drives from various organizations and from personal contributions.  These monies are placed in the Veterans Home Benefit Fund.   

                                             DEPARTMENT OF MILITARY AFFAIRS AND VETERANS SERVICES 
               #C.         Authorize to accept donations of a golf cart, laptops, software and docking station for computers, computer services, benches, handicap door access, and brick and paver engraving, in the amount of $16,990.90 for the NH State Veterans Cemetery to be used for the operation, maintenance, and improvement of the cemetery. 

                                             DEPARTMENT OF ADMINISTRATIVE SERVICES 
               #D.         Authorize to accept direct reimbursement for professional development, green purchasing technical assistance, and cooperative purchasing education from the National Association of State Procurement Officials, for an amount not to exceed $51,200.  Effective upon G&C approval through December 31, 2020.

               #A.         Authorize the nomination of Captain Michael C. Allen, Derry, NH, for promotion to the rank of Major, NH Air National Guard.  This officer meets all prerequisites for this grade, having been found to be physically, mentally, morally, and professionally qualified.

               #A.         Authorize the Division of Fire Standards and Training and Emergency Medical Services to enter into a sole source no-cost amendment to the grant agreement with the City of Concord (originally approved by G&C on 10-2-19, item #43), for the purpose of expanding their mobile integrated healthcare program, called NH Project FIRST, to include several additional communities.  Effective upon G&C approval through September 29, 2020.  100% Federal Funds.

                              Bureau of Facilities Maintenance and Office Services 
#6           Authorize to retroactively enter into a lease amendment with the City of Dover, NH, for continued occupation of office space at 61 Locust Street, Suites 128 and 129 in Dover, for the Department’s Juvenile Probation and Parole Officers, by increasing the price in the amount of $50,078.88 from $116,867.48 to $166,946.36 and extending the completion date from September 30, 2019 to September 30, 2021.  Effective upon G&C approval.  49% General, 51% Federal Funds.   

                              Division of Public Health Services 
#7           Authorize to amend existing agreements with the vendors as detailed in letter dated December 26, 2019 (originally approved by G&C on 6-19-19, item #78E & 9-18-19, item #25), for the provision of Regional Public Health Network services, statewide, by increasing the price by $197,543 from $8,946,753 to $9,144,296 with no change to the completion date of June 31, 2021.  100% Federal Funds.   

#8           Authorize to enter into a sole source agreement with LabLogic Systems Inc., Tampa, FL, to provide repair and maintenance services for the Hidex 300 SL in an amount not to exceed $22,050.  Effective upon G&C approval through June 30, 2022.  100% Other Funds.   

                              Division of Economic & Housing Stability 
#9           Authorize to enter into a sole source agreement with the University of NH, Durham, NH, to provide nutrition education services to individuals who are eligible for Supplemental Nutrition Assistance Program benefits, in an amount not to exceed $2,087,332.  Effective retroactive to October 1, 2019 through September 30, 2021.  100% Federal Funds.   

                              Division for Children, Youth & Families 
#10        Authorize to enter into a sole source agreement with the University of NH, Durham, NH, to provide training on Implementation Science framework in an amount not to exceed $9,950.  Effective upon G&C approval through June 15, 2020.  50% Federal, 50% General Funds.   

#11        Authorize to amend an existing sole source contract with Casey Family Programs Inc., Seattle, WA), a foreign, non-profit corporation (originally approved by G&C on 6-7-17, item #11B), to assess community level risk information in collaboration with the DCYF and the Division of Public Health Services at no cost to the Department, extend the completion date retroactively from December 31, 2019 to December 31, 2022. 

#12        Authorize to accept and expend the additional amount of $12,301 from the Family Violence Prevention & Services grant award for domestic violence prevention services.  Effective upon G&C approval through June 30, 2020.   

                              New Hampshire Hospital 
#13        Authorize to enter into a sole source agreement with Riverbend Community Mental Health Inc., Concord, NH, to provide electroconvulsive therapy treatments, in an amount not to exceed $60,000.  Effective upon G&C approval through December 31, 2021, with the option to renew contracted services for up to 4 additional years.  66% Other (Interagency and Agency Income), 34% General Funds.   

                              DEPARTMENT OF TRANSPORTATION 
#14        Authorize the Bureau of Right of Way to pay property owners $65,420 as documented in the Contemplated Awards List, for amounts greater than $5,000 for the period extending from December 26, 2019 through January 8, 2020.   

#15        Authorize the Bureau of Rail & Transit to pay the Manchester Transit Authority, Manchester, NH, an amount not to exceed $86,958 to provide a portion of the non-Federal Funds required to match Federal Transit Administration operating funds.  Effective upon G&C approval through June 30, 2021.  100% General Funds.   

#16        Authorize the Bureau of Rail & Transit to pay the Cooperative Alliance for Seacoast Transportation, Dover, NH, an amount not to exceed $69,566 to provide a portion of the non-Federal Funds required to match Federal Transit Administration operating funds.  Effective upon G&C approval through June 30, 2021.  100% General Funds.   

#17        Authorize the Bureau of Rail & Transit to pay the City of Nashua, NH, an amount not to exceed $69,566 to provide a portion of the non-Federal Funds required to match Federal Transit Administration operating funds.  Effective upon G&C approval through June 30, 2021.  100% General Funds.   

#18        Authorize the Bureau of Rail & Transit to enter into a contract amendment with the Tri-County Community Action Program, Berlin, NH (originally approved by G&C on 5-15-19, item #37K), to provide a portion of the non-Federal Funds required to match Federal Transit Administration operating funds, by increasing the amount by $34,780 from $990,535 to $1,025,315.  Effective upon G&C approval through June 30, 2021.  100% General Funds.   

#19        Authorize the Bureau of Bridge Design to enter into an agreement with Becker Structural Engineers Inc., for the preliminary design, public involvement process, associated environmental & cultural services, final design, and construction support services for rehabilitation of the exiting bridge carrying NH Route 49 over Mad River in the Town of Thornton, for a total amount not to exceed $302,900.  Effective upon G&C approval through December 31, 2022.  100% Other Funds (SB 367 Funds).   

#20        Authorize the Bureau of Construction to enter into a contract with J Hutchins Inc., Richmond, VT, for 7.4 miles of pavement resurfacing in Colebrook and Stewartstown, on the basis of a low bid of $1,807,706.50.  (2)Further authorize a contingency in the amount of $90,385.33 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through August 28, 2020.  100% Federal Funds.   

#21        Authorize to accept and place on file the Bureau of Construction’s year end summary report of active projects that may require contingency use, for 2019.   

#22        Authorize the Bureau of Construction to enter into a contract with C.W. Sliter & Sons LLC d/b/a CWS Fence & Guardrail Co., Andover, NH, for guardrail replacement in Districts 4 and 5, on the basis of a low bid of $839,188.25.  (2)Further authorize a contingency in the amount of $41,959.41 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through October 23, 2020.  100% Federal Funds.   

#23        Authorize the Bureau of Construction to enter into a contract with GMI Asphalt LLC of Belmont, NH, for resurfacing various Tier 2 roadways in District 2 in Grafton and Sullivan Counties, on the basis of a low bid of $2,123,488.80.  (2)Further authorize a contingency in the amount of $106,174.44 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through September 18, 2020.  100% Federal Funds.   

                              OFFICE OF THE GOVERNOR 
#23A     Authorize to amend the approved salary labor grade for David G. Parenteau, of Manchester, NH, who was appointed Interim Police Standards and Training Director at the December 18, 2019 Governor and Councilor meeting, by placing his annual salary at Labor Grade GG, Step 2 $82,699.24, from Labor Grade GG, Step 1 $77,609.48, for the period effective retroactive to December 18, 2019 through February 15, 2020. 

                              OFFICE OF STRATEGIC INITIATIVES 
#24        Authorize to enter into a contract agreement with the Manchester Transit Authority, Manchester, NH, to utilize funding from the Volkswagen Environmental Mitigation Trust to partially fund the purchase and replacement of MTA diesel school buses to reduce air pollution, in the amount not to exceed $750,000.  Effective upon G&C approval through June 30, 2021.  100% Volkswagen Funds.   

                              NEW HAMPSHIRE EMPLOYMENT SECURITY 
#25        Authorize to retroactively enter into a sole source contract amendment with the Center for Employment Security Education and Research Inc., Washington, DC (originally approved by G&C on 11-14-18, item #42), for the implementation of the Workforce Connect software suite, by extending the completion date from December 31, 2019 to April 30, 2020.  100% Federal Funds.   

                              NEW HAMPSHIRE INSURANCE DEPARTMENT 
#26        Authorize to enter into a contract with Public Consulting Group Inc., Boston, MA, for consulting services, in the amount of $69,500.  Effective upon G&C approval through December 31, 2020.  100% Other Funds.   

                              DEPARTMENT OF CORRECTIONS 
#27        Authorize to enter into a sole source contract agreement with Virginia Ross, Hampton, NH, for Recovery Coach training and professional services, in the amount of $14,310.  Effective upon G&C approval through August 31, 2020.  100% Agency Income.   

                              NEW HAMPSHIRE LIQUOR COMMISSION 
#28        Authorize to enter into a sole source contract with Evenica Corp., Burlington, Ontario, to complete design development and deployment, and to provide follow-up support and maintenance of the Commission’s Business to Consumer and Business to Business eCommerce initiative in an amount not to exceed $859,600.  Effective upon G&C approval through July 31, 2023.  100% Liquor Funds.   

                              OFFICE OF THE CONSUMER ADVOCATE 
#29        Authorize to continue membership and participation in the National Association of State Utility Consumer Advocates, in the amount of $4,111.  Effective retroactive to January 1, 2020 through December 31, 2020.  100% Other Funds (Utilities Assessments).   

                              NEW HAMPSHIRE FISH AND GAME DEPARTMENT 
#30        Authorize to acquire the fee title to 123+/- acres in the Town of Sutton, NH, from the Law Office of Mark R. Dunn, on behalf of Daniel Holland, at the fair market value of $110,000.  Effective upon G&C approval through March 31, 2020.  100% Federal Funds.   

#31        Authorize to pay the Northeast Association of Fish and Wildlife Agencies, Petersburgh, NY, $3,000 annual membership dues.  Effective retroactive to January 1, 2020 through December 31, 2020.  100% Fish and Game Funds.   

                              DEPARTMENT OF BUSINESS AND ECONOMIC AFFAIRS 
#32        Authorize the Office of International Commerce to retroactively renew membership with The Council of State Governments d/b/a Eastern Trade Council, New York, NY, to provide regional export trade research and related trade development programs, in the amount of $5,000.  Effective July 1, 2019 through June 30, 2020.  100% General Funds.   

                              DEPARTMENT OF NATURAL AND CULTURAL RESOURCES 
#33        Authorize the Division of Parks and Recreation (DPR) to enter into a memorandum of understanding with the NH Liquor Commission (NHLC) for DPR to sell preloaded State Park gift cards to NHLC for the purpose of being sold to customers at NHLC locations.  Effective upon G&C approval through December 31, 2022.   

#34        Authorize the NH State Library to renew membership with the Chief Officers of State Library Agencies, Lexington, KY, in the amount of $5,000.  Effective retroactive to January 1, 2020 through December 31, 2020.  45% General, 55% Federal Funds.   

#35        Authorize the Division of Forests and Lands to exchange, with the Town of Hopkinton, at no cost to the State, 30.3+/- acres of state-owned land known as “Contoocook State Forest” in Hopkinton, NH for a 97 acre +/- parcel of land known as the “Hopkinton Town Forest” that abuts Mast Yard State Forest.  No State Funds.  (2)Further authorize to accept title by Warranty Deed, at no cost to the State, from the Town of Hopkinton a 97-acre +/- parcel of land known as the “Hopkinton Town Forest”, abutting Mast Yard State Forest, in the Town of Hopkinton.  No State Funds.  (3)Further authorize to accept a $1,100 administrative fee from the Town of Hopkinton.   

#36        Authorize the Division of Historical Resources to award a Conservation Number Plate (Moose Plate) Grant to the Town of Belmont, NH, in the amount of $10,000 for masonry repairs and brick repointing at the Belmont Public Library.  Effective upon G&C approval through September 30, 2021.  100% Other Funds (Agency Income).   

                              DEPARTMENT OF ENVIRONMENTAL SERVICES 
#37        Authorize to amend an agreement with Rockingham Planning Commission, Exeter, NH (originally approved by G&C on 5-1-19, item #55), for the Country Pond Watershed plan development: Phase 1 Planning for the West Watershed, Direct Drainage, Southeast Watershed, and Cedar Swamp Watershed project, by increasing the amount by $800 from $64,000 to $64,800, and by extending the expiration date from December 31, 2020 to June 30, 2021.  100% Federal Funds.   

#38        Authorize to award a sole source grant to the Gundalow Company, Portsmouth, NH, in the amount of $25,000 to conduct an on-board environmental education series at locations in the Seacoast region.  Effective upon G&C approval through December 31, 2020.  100% Federal Funds.   

#39        Authorize to enter into a grant agreement with West Alton Marina LLC, Alton Bay, NH, in the amount of $10,500 for an expansion of the boat sewage pumpout station and the subsequent operation and maintenance costs for the pumpout station.  Effective upon G&C approval through March 31, 2024.  100% Federal Funds.   

#40        Authorize to award a loan to the City of Manchester, NH, in the amount not to exceed $13,000,000 to finance the Merrimack River Water Treatment Plant Project.  Effective upon G&C approval through December 1, 2022.  100% Drinking Water and Groundwater Trust Fund.   

#41        Authorize to enter into two Easement Deeds, one with Mr. Donald Buskey of Swanzey, NH, and the other with Messrs. Richards and James French of Barnstable, MA, to provide rights of access needed to reconstruct the state-owned Weeks Crossing Dam in Warren, NH. 

#42        Authorize to enter into a contract with Carollo Engineers Inc., Walnut Creek, CA, in the amount of $120,000 to provide expert technical assistance consulting services for water reuse demonstration project on Star Island.  Effective upon G&C approval through June 30, 2021.  100% Clean Water Revolving Loan Management Funds.   

#43        Authorize to enter into an agreement with 120WaterAudit Inc., Zionsville, IN, in order to provide services including the distribution of water filter pitchers and filter cartridges to at-risk mothers and infants exposed to arsenic in private water supply wells, in the amount of $280,250.  Effective upon G&C approval through December 31, 2025.  100% Drinking Water/Groundwater Trust Fund.   

#44        Authorize to award state aid grant funds for wastewater facility projects for the five entities as detailed in letter dated January 17, 2020, in the aggregate amount of $5,861,066.  Effective upon G&C approval.  100% General Funds.   

#45        Authorize Keewaydin Park Association’s request to perform work on Lake Winnipesaukee in Alton Bay. 

#46        Authorize Hillary Johnston Trust’s request to amend wetlands permit #2018-03472 (originally approved by G&C on 7-10-19, item #54), to perform work on Lake Winnipesaukee in Moultonborough. 

                              DEPARTMENT OF EDUCATION 
#47        Authorize the Office of Social & Emotional Wellness to enter into a sole source cooperative project agreement with Keene State College, Keene, NH, in an amount not to exceed $1,350,000 to work in partnership with the NH Department of Education, to provide external evaluation services of the project AWARE 2019 Grant.  Effective upon G&C approval through September 29, 2025.  100% Federal Funds.   

                              DEPARTMENT OF SAFETY 
#48        Authorize the Division of Homeland Security and Emergency Management to enter into a grant agreement with the Town of Goffstown, for a total amount of $9,999.75 to update the local hazard mitigation plan.  Effective upon G&C approval through April 1, 2022.  100% Federal Funds.   

#49        Authorize the Division of Homeland Security and Emergency Management to retroactively accept and expend grant funds form the Department of Safety, Office of the Commissioner, in an amount not to exceed $70,000 for providing a hosted web-based cellular phone notification system and associated services for HSEM to alert and warn the public of any impending dangers.  Effective July 1, 2019 through June 30, 2021.  100% Agency Income.   

#50        Authorize the Division of State Police to accept and expend $23,934 from the Department of Safety, Office of Highway Safety, titled “NHSP Distracted Driving Patrols” for the purpose of encouraging all motor vehicle operators to focus on the task of driving.  Effective upon G&C approval through September 30, 2020.  100% Agency Income.   

#51        Authorize the Division of Homeland Security and Emergency Management to accept and expend grant funds from the Department of Safety, Office of the Commissioner, in an amount not to exceed $37,800 to fund two existing part-time, non-classified program positions to assist with drafting the Statewide Comprehensive Swift Water Plan.  Effective upon G&C approval through August 31, 2021.  100% Agency Income.   

#51A     Authorize the Division of Motor Vehicles to enter into a retroactive lease agreement with KGI Gorham LLC, Providence, RI, for 5,500 square feet of office space for the Division of Motor Vehicles office located in Gorham, NH, for an amount not to exceed $261,250.  Effective November 1, 2019 through October 31, 2024.  100% Agency Income (Cost of Collections).   

                              DEPARTMENT OF ADMINISTRATIVE SERVICES 
#52        Authorize to accept and place on file the report containing a list of vendors that agencies have contracted with for the period of October 1, 2019 through December 31, 2019.   

#53        Authorize the Division of Public Works Design and Construction to enter into an agreement with Richardson Engineering PLLC, Berwick, ME, for electrical services required for planning, design and construction of various Public Works’ Projects as necessary and required by the Department, in an amount not to exceed $300,000.  Effective upon G&C approval through June 30, 2022. 

#54        Authorize, acting on behalf of the NH Fish and Game Department, to sell the State-owned land, buildings, and other improvements known as Brentwood Game Farm, Brentwood, NH, to Game Farm LLC for $535,000 plus the $1,100 administrative fee.  Effective upon G&C approval.  100% Net Proceeds from Sale (Fish and Game Fund).   

#55        Authorize the Division of Personnel’s request for waiver of classification decisions.  Effective upon G&C approval.
