#A1. Authorize to enter into an amendment with Tri-County Community Action Program to increase service rates and complete planning activities for new construction or renovations to an existing building owned by the Contractor. This increases the price limitation by $92,000 from $11,940,600 to an amount not to exceed $12,032,600, effective upon G&C approval. There is no change to the completion date of June 30, 2017. 64.5% Federal, 21.5% General, and 14% Other Funds.
#A2. Authorize to amend existing individual agreements retroactively and prospectively with the state’s two managed care health plans listed in the letter dated June 14, 2016. Effective retroactive to January 1, 2016 in the case of New Hampshire Protection Program’s Transitional Population and in all other cases. Effective July 1, 2016, upon approval of the G&C through June 30, 2017.
FISH AND GAME DEPARTMENT
#B. Authorize to enter into a grant agreement with the Town of Milford for $50,000 to provide partial funding for the Osgood Pond Restoration Project in Milford, NH. Effective upon G&C approval through July 30, 2017. Funding is 100% Fisheries Habitat Funds.
