#6 Authorize to enter into a retroactive sole source amendment to the existing lease with 75 New Hampshire LLC, c/o James J. Horne, CPManagement Inc., Exeter, NH (originally approved by G&C on 12-9-09, item #79), for continued occupation by the Seacoast District Office, by increasing the price in the amount of $628,992 to $4,127,187.12 from $3,498,195.12, and by extending the term from March 31, 2017 to May 31, 2018. Effective retroactive to April 1, 2017. 60% General, 40% Federal Funds.

#7 Authorize to enter into a new lease with 19 Rye St, LLC, c/o James J. Horne, CPManagement Inc., Exeter, NH, for office space in Portsmouth in the amount of $7,511,560. Effective April 20, 2017 through March 31, 2018. Occupancy of the premises and commencement of rental payments shall be June 1, 2018 upon satisfactory completion of renovations.

Office of Medicaid Business and Policy
#8 Authorize to retroactively continue an increase in hourly rate by 30% for recruitment and retention purposes for the Pharmaceutical Service Specialist classification, position number 12433, as approved by G&C on March 25, 2015, item #9. Effective February 26, 2017 through March 14, 2019, which is two years from the date of Division of Personnel approval.

Division for Behavioral Health
#9 Authorize New Hampshire Hospital to enter into a sole source agreement with Johnson Controls Inc., Milwaukee, WI, for the provision of two extended ten year warranties for two YORK YMC2 Water-Cooled Magnetic Bearing Centrifugal Chillers (temperature control systems) installed at New Hampshire Hospital, in an amount not to exceed $87,449. Effective upon G&C approval through June 30, 2026. 100% General Funds.

#9A Authorize the Bureau of Mental Health Services to establish a consultant list of six attorneys and enter into agreements with them as necessary to provide legally required representation for certain individuals admitted to the NH Hospital at a price not to exceed $60,800. Effective July 1, 2017 or upon G&C approval, whichever is later, through June 30, 2019. 100% General Funds.

#10A Authorize to make a retroactive one-time payment from SFY 2017 funds to Joan Brossia, Case Technician I, Child Support Services, in the amount of $1,586.13 to compensate for wages that were not paid for SFY 2016. 34% General, 66% Federal Funds.

Division of Public Health Services
#11 Authorize to amend a sole source contract with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 12-20-13, item #53), to continue to identify the needs, provide implementation, and evaluation of tasks to assist New Hampshire’s 13 small rural hospitals with improving their telehealth and telemedicine capabilities, and improving quality performance through Lean or other quality improvement training and projects, by increasing the price by $39,151 from $446,940 to an amount not to exceed $486,091. Effective upon G&C approval through August 31, 2017. 100% Federal Funds.

DEPARTMENT OF TRANSPORTATION
#12 Authorize the Bureau of Right of Way and Bureau of Highway Maintenance to pay property owners $108,000 as documented in the Highway Maintenance Damage Awards List for amounts greater than $5,000 for the period extending from May 12, 2016 through March 8, 2017.

#13 Authorize the Bureau of Highway Maintenance, Well Section, to enter into a contract with Capital Well Company Inc., Dunbarton, NH, for a 6-inch drilled well and pump on the property of Thomas Schmit, Chester, NH, in the amount of $20,483. Effective upon G&C approval through June 30, 2017. 100% Highway Funds.

#14 Authorize the Bureau of Highway Maintenance, Well Section, to enter into a contract with Capital Well Company Inc., Dunbarton, NH, for a 6-inch drilled well and pump on the property of Donna Cronin, New Ipswich, NH, in the amount of $17,607. Effective upon G&C approval through June 30, 2017. 100% Highway Funds.

COMMUNITY DEVELOPMENT FINANCE AUTHORITY
#15 Authorize to award a grant to the Town of Gilford, NH, in the amount of $300,000 for the purpose of assisting Laconia Area Community Land Trust with the acquisition and site improvements to support the construction of a 24 unit affordable senior housing development known as Gilford Village Knolls III to be located at 41 Potter Hill Road, Gilford, NH. Effective April 5, 2017 through December 31, 2018. 100% Federal Funds.

DEPARTMENT OF INFORMATION TECHNOLOGY
#17 Authorize, for the benefit of the Department of Safety, to enter into a sole source contract extension to a contract with MultiProcess Computer Corporation, Windham, NH (originally approved by G&C on 6/21/06, item #18B), for document imaging, support and maintenance services for Financial Responsibility Imaging System software, by increasing the amount by $50,000 from $642,500 to $692,500, and by extending the end date from June 30, 2017 to June 30, 2018. 100% Other Funds. The Agency Class 027 used by the Department of Safety to reimburse DoIT for this maintenance service is 100% Agency Income – Cost of Collections.

DEPARTMENT OF AGRICULTURE, MARKETS & FOOD
#18 Authorize the NH State Conservation Committee to enter into a grant with Southeast Land Trust of NH, Exeter, NH, in the amount of $20,000 for the Stonehouse Forest, in the town of Barrington, Strafford County. Effective upon G&C approval through December 1, 2018. 100% Other Funds – State Conservation Committee.

NEW HAMPSHIRE EMPLOYMENT SECURITY
#19 Authorize to extend the positions as detailed in letter dated March 10, 2017, in a temporary status for one year. The estimated fiscal impact to SFY 2017-2018 to extend these positions is $182,970.60.

#20 Authorize to enter into a sole source contract amendment with SymbioSys Solutions Inc., Nashua, NH (originally approved by G&C on 6-22-11, item #60), to extend maintenance and support of the NH Unemployment Insurance System, by increasing the amount by $1,228,820 from $16,159,130 to $17,387,950, and by extending the end date beginning January 1, 2018 through June 30, 2018.

DEPARTMENT OF CORRECTIONS
#21 Authorize to enter into a contract with Eric Joyce d/b/a Joyce Dental Laboratory, Laconia, NH, to provide Dental Laboratory Services for the NH Department of Corrections, in the amount of $142,660. Effective July 1, 2017 through June 30, 2019, with the option to renew for one additional period of up to two years. 100% General Funds.

DEPARTMENT OF RESOURCES AND ECONOMIC DEVELOPMENT
#22 Authorize the Division of Parks and Recreation to enter into a sole source contract with the Student Conservation Association Inc., Charlestown, NH, to provide interpretive and conservation services at various State Parks, in the amount of $520,145. Effective upon G&C approval through June 30, 2019. 100% Agency Income.

NEW HAMPSHIRE FISH AND GAME DEPARTMENT
#23 Authorize to acquire fee title to 1+/- acres in Enfield, NH from Gregory Baker, at the fair market value of $48,000. Effective upon G&C approval through June 30, 2017. 100% Federal Funds.

#24 Authorize to acquire fee title to 149+/- acres in Londonderry, NH, from the Southeast Land Trust of NH for $650,000. Effective upon G&C approval through June 30, 2017. 30% Federal, 70% Other (Wildlife Habitat Funds).

#26 Authorize to enter into a sole source contract with HYSR, Dartmouth, MA, to complete the QPPQ Proof of Concept Project for the purpose of enhancing the QPPQ Transform Method to calculate long-period daily stream flows at ungagged river locations in NH, in the amount of $30,000. Effective upon G&C approval through September 30, 2017. 100% General Funds.

#27 Authorize to enter into a memorandum of agreement with the NH Department of Health and Human Services, and the NH Department of Agriculture, Markets and Food, to support the development and distribution of Integrative Pest Management for Tick-Borne Disease Prevention Education Kits, in the amount of $4,000. Effective upon G&C approval through September 30, 2018. 100% Federal Funds.

#28 Authorize to enter into a grant agreement with Lady Tracey Ann Inc., Seabrook, NH, to partially fund the installation of two diesel marine engines, in an amount not to exceed $41,432. Effective upon G&C approval through September 30, 2017. 100% Federal Funds.

#29 Authorize to pay calendar year 2017 membership dues to the Association of State Dam Safety Officials, Lexington, KY, in the amount of $2,817. Effective retroactive to January 1, 2017 through December 31, 2017. 29% Dam Safety, 71% General Funds.

#30 Authorize to enter into an agreement with the Rockingham Planning Commission, Exeter, NH, to complete the Collaborative Engagement and Capacity Building in the Powwow River Watershed-Phase I, in the amount of $15,880. Effective upon G&C approval through December 31, 2018. 100% Federal Funds.

#31 Authorize to enter into an agreement with the Lakes Region Planning Commission, Meredith, NH, to complete the Development of a Septic System Assessment Model – Phase I, in the amount of $15,010. Effective upon G&C approval through December 31, 2018. 100% Federal Funds.

DEPARTMENT OF SAFETY
#35 Authorize the Division of State Police to enter into a contract with East Coast Welding, Gilford, NH, for welding services for the repair of boats, trailers and other marine-related equipment at Marine Patrol in Gilford, NH, in an amount not to exceed $25,500. Effective July 1, 2017 to June 30, 2020, with an option to extend for one two-year period at the State’s sole discretion. 100% Federal Funds.

#36 Authorize the Division of Emergency Services and Communications to enter into a contract with Dr. James Suozzi LLC, for the purpose of providing consulting services as the Division’s Medical Director, in the amount of $41,500. Effective upon G&C approval through June 30, 2019, with an option to renew for two additional years at the sole discretion of the State. 100% Agency Income.

#36A Authorize the Division of State Police to accept and expend an additional $40,000 of federal funds from the US Department of Justice, Office of Justice Programs, National Institute of Justice for the purpose of reducing DNA Backlog cases within the State Police Forensic Lab. Effective upon G&C approval through June 30, 2017. 100% Federal Funds.

DEPARTMENT OF JUSTICE
#37 Authorize to enter into two sole source contracts for a grand total of $26,000 with Dr. Margaret Greenwald, Hallowell, ME, as an additional medical examiner from April 17, 2017 to April 21, 2017, for a total not to exceed $13,000 and Dr. Kristin Roman, Staten Island, NY, as an additional medical examiner from May 22, 2017 to May 26, 2017, for a total not to exceed $13,000 to aid the Chief Medical Examiner and the Deputy Chief Medical Examiner. Effective upon G&C approval through June 30, 2017. 100% Federal Funds.

DEPARTMENT OF ADMINISTRATIVE SERVICES
#38 Authorize the Division of Public Works Design and Construction to enter into a contract with Meridian Construction Corp., Gilford, NH, for the NH Liquor Commission Renovation of Warehouse space to Office Space, Concord, NH, for a total price not to exceed $1,985,900. Effective upon G&C approval through December 1, 2017. 87% Capital-Other, 13% Other Funds (Store Operations Funds). (2)Further authorize the amount of $50,000 for payment to the Department of Administrative Services, Division of Public Works Design and Construction, for engineering services provided, bringing the total to $2,035,900. 100% Other Funds (Store Operations Funds).

#39 Authorize the Division of Public Works Design and Construction to enter into a contract with Surveillance Specialties LTD, Wilmington, MA, for the Glencliff Home Campus Security Upgrades, Benton, NH, for a total price not to exceed $313,532. (2)Further authorize a contingency in the amount of $45,000 for unanticipated expenses, bringing the total to $358,532. (3)Further authorize the amount of $20,000 for payment to the Department of Administrative Services, Division of Public Works Design and Construction, capital clerk for oversight and engineering services provided, bringing the total to $378,532. Effective upon G&C approval through August 31, 2017. 100% Capital-General Funds.

#40 Authorize the Division of Personnel’s request for waiver of classification decisions. Effective upon G&C approval.

#41 Authorize the Division of Public Works Design and Construction to enter into a contract with North Branch Construction Inc., Concord, NH, for the NH Army National Guard Field Maintenance Shop located in Hooksett, NH, for a total price not to exceed $9,919,000. (2)Further authorize a contingency in the amount of $496,000 for unanticipated structural expenses or owner initiated changes, bringing the total to $10,415,000. Effective upon G&C approval through December 14, 2018. 100% Federal Funds.

#43 Authorize to exercise an option to retroactively amend the State’s contract with James R. St. Jean Auctioneer’s, Epping, NH (originally approved by G&C on 10-1-14, item #57), to continue to allow the use of Live Auction Service, by extending the end date from March 31, 2017 to June 30, 2017.

DEPARTMENT OF STATE
#44 Authorize, to confirm, per RSA 21-G:29, the Secretary of State’s nomination of Justin Cutting (30 Moulton Road, Hampton NH 03842) to serve on the Executive Branch Ethics Committee. Mr. Cutting’s term will begin on July 1, 2017. He will replace James A. Normand, who is not seeking reappointment. Mr. Normand has served on the committee faithfully and diligently for the past seven years.
