#6A      Authorize to amend Fiscal Committee item #FIS 22-043, approved on January 21, 2022 and G&C on January 26, 2022, item #13A, by accepting funds from the NH Liquor Commission in the amount of $833,613 to return Federal funds from the American Rescue Plan Act, which were used to purchase At-Home COVID-19 test kits and redistribute unsold tests at no cost. #6B      Authorize the Bureau of Facilities and Office Services to enter into a sole source lease with the Concord Litho Group, Inc., Concord, NH, for warehouse space, for the Departments Emergency Services Unit storage of COVID-related supplies and equipment in the amount not to exceed $3,959,228. Effective June 1, 2022 through January 31, 2027. 100% Federal Funds.

                             Division of Finance and Procurement
#7        Authorize the Division of Medicaid Services to accept and expend federal funds in the amount of $5,672,079 from the Centers for Medicare and Medicaid Services to fund the NH Hospital Disproportionate Share Hospital Payments. 100% Federal Funds (2) Further authorize to accept and expend an additional Intra-Agency transfer from the Disproportionate Share Hospital Program in the amount of $5,666,413 which will be used to fund several projects at NH Hospital and supplant Other funds (Provider and Café fees) which are anticipated to be less than budgeted.                              Division of Medicaid Services
#8        Authorize to amend an existing contract with Health Services Advisory Group, Inc., Phoenix, AZ, (originally approved by G&C on 6/19/19, item #15), to continue providing external quality reviews of the Department’s contracted managed care plans to improve the Managed Care Organization’s performance with respect to quality, timeliness, and access to care for NH Medicaid beneficiaries enrolled in Medicaid Care Management statewide, by exercising a contract renewal option by increasing the price limitation by $1,764,774 from $2,464,200 to $4,228,974 and extending the completion date from June 30, 2022 to June 30, 2024. Effective July 1, 2022. 75% Federal, 17% General, and 8% Other Funds.

#8A      Authorize to enter into a contract with Milliman, Inc, Seattle, WA, in the amount of $5,316,530 for actuarial services and technical support for the Department’s Division of Medicaid Services. Effective upon G&C approval for July 1, 2022, whichever is later, through June 30, 2024, with the option to renew for up to 4 additional years. 50% Federal, 35% General, 15% Other Funds. 

                        Division of Public Health Services
#9        Authorize to accept and expend funds from the Health Resources and Services Administration in the amount of $46,166, to fund the Pediatric Mental Health Care Access Program. Effective upon G&C approval through June 30, 2023. 100% Federal Funds.

#10      Authorize to amend existing contracts with the Contractors listed in letter dated May 2, 2022, for the provision of regional Public Health Network services statewide, (originally approved by G&C on 6/19/19, item #78E), by increasing the total price limitation by $339,452 from $20,248,600 to $20,588,052 with no change to the contract completion date. Effective upon G&C approval. 100% Federal Funds.

#11      Authorize to accept and expand funds from the Council for State and Territorial Epidemiologists to implement a Social Determinants of Health questionnaire supplement in the amount of $30,000. Effective upon G&C approval through June 30, 2023. 100% Other Funds.

#12      Authorize the Bureau of Informatics, Behavioral Risk Factor Surveillance program to accept and expend other funds in the amount of $40,500 from the Alzheimer’s Association, $10,000 from the University of NH Institute on Disability and $15,000 from the University of NH Disability and Health Program to fund the Behavioral Risk factor Surveillance program. #13      Authorize to enter into an amendment to an existing Cooperative Project Agreement with the University of NH, Durham, NH, (originally presented to G&C on 12/20/20, informational item H), to continue to provide COVID-19 sample testing, by increasing the price limitation by $361,147 from $8,182,500 to $8,543,647 and with no change to the project completion date of December 31, 2022. Effective retroactive to December 22, 2021. 100% Federal Funds. 

#14      Authorize to amend an existing contract with New Hampshire Harm Reduction Coalition, Dover, NH, (originally approved by G&C on 8/26/20, item #17), for harm reduction services in Syringe Services Programs in order to reduce the rate of opioid misuse and infectious disease complications associated with opioid use, by exercising a contract renewal option by increasing the price limitation by $1,341,246 from $770,000 to $2,111,246 and by extending the completion date from August 31, 2022 to August 31, 2023. Effective upon G&C approval. 100% Federal Funds.

#15      Authorize to amend an existing contract with bioMerieux, Inc., Durham, NC, (originally approved by G&C on 7/10/19, item #9), to provide repair and service for Vidas and Vitek laboratory instruments located at the Public Health Laboratories, by exercising a renewal option by increasing the price limitation by $105,123 from $96,632 to $201,755 and by extending the completion date from July 10, 2022 to July 10, 2025. 100% Federal Funds.

#16      Authorize to amend an existing contract with Community Health Access Network, Newmarket, NH, (originally approved by G&C on 1/23/19, item #29), to continue to improve the prevention and management of cancer, arthritis, diabetes, and heart disease by exercising a contract renewal option by increasing the price limitation by $2,166,000 from $2,192,078 to $4,358,078 and by extending the completion date from June 30, 2022 to June 30, 2024. Effective upon G&C approval 95% Federal, 5% General Funds.

                        Division of Long Term Supports and Services
#17      Authorize to enter into a sole source contract with American Association on Intellectual and Development Disabilities, Silver Spring, MD, in the amount of $379,592 for Supports Intensity Scale assessments for individuals receiving services through the Developmental Services System. Effective upon G&C approval through December 31, 2022, with the option to renew for up to 1 additional year. 50% Federal, 50% General Funds.

#18      Authorize to amend an existing contract with As Life Goes on, LLC, Hollis, NH, to continue providing behavioral health long term care bed capacity exclusively for patients transferring from NH Hospital of Glencliff Home, by increasing the price limitation by $45,000 from $2,095,416 to $2,140,416, with no change to the shared price limitation of $520,416, and by extending the completion date from May 31, 2022 to May 31, 2023. Effective retroactive to March 8, 2022. 100% Federal Funds. 

#19      Authorize to amend existing contracts with the Contactors listed in letter dated April 20, 2022, (originally presented to G&C on 6/24/20, informational item O), to continue providing adult day program services, by increasing the total price limitation by $1,308,150 from $1,508,940 to $2,817,090 and by extending the completion date from June 30, 2022, to June 30, 2024. Effective upon G&C approval. 56% Federal, 44% General Funds.

                        Division for Behavioral Health
#20      Authorize to accept and expend funds from the Substance Abuse and Mental Health Services Administration, entitled NH’s strategy to address overall capacity, consistency, and quality of 988 services, in the amount of $ 338,302. Effective upon G&C approval through June 30, 2023. 100% Federal Funds.  Contingent upon Fiscal Committee approval on May 20, 2022.

#21      Authorize to enter into a sole source amendment to an existing contract with RCR Technology Corporation, Inc., Indianapolis, IN, (originally approved by G&C on 7/10/13, item #57), to continue to provide service and maintenance to the NH Child and Adolescent Needs and Strengths Assessment and the NH Adult Needs and Strengths Assessment data management system, to continue to provide funds to maintain community provider access, and to continue providing support for staff certification services linked to the Contractor’s data management system, by increasing the price limitation by $391,527 from $1,235,423.50 to $1,626,950.50, and by extending the completion date from October 31, 2022 to October 31, 2024. Effective upon G&C approval. 34.18% Federal, 65.82% General Funds.

                        Governor’s Commission on Alcohol & Other Drugs
#22      Authorize to amend an existing contract with Granite United Way, Concord, NH, (originally approved by G&C on 12/16/20, item #13), to continue providing Recovery Friendly Workforce Initiative services that assist businesses with promoting employee health and wellness for by proactively preventing substance misuse and supporting recovery from substance use disorders, by exercising a contract renewal option by increasing the price limitation by $400,000 from $900,000 to $1,300,000 and extending the completion date from June 30, 2022 to June 30, 2023. Effective upon G&C approval or July 1, 2022 whichever is later. 100% Other Funds (Governor’s Commission).

#23      Authorize to amend an existing contract with Greater Seacoast Community Health, Somersworth, NH, (originally approved by G&C on 1/22/21, item #23), for Family Support Coordinator service statewide, by exercising a contract renewal option by increasing the price limitation by $75,000 from $150,000 to $225,000 and extending the completion date from June 30, 2022 to June 30, 2023. Effective upon G&C approval or July 1, 2022 whichever is later. 100% Other Funds (Governor’s Commission).

#24      Authorize to amend an existing contract with Genesis Eldercare Rehabilitation Services, LLC, Kennett Square, PA, (originally approved by G&C on 6/30/21, item #9), to continue providing physical and occupational therapy services to residents and to maintain and extend their license for use of premises of the Glencliff Home, by exercising a renewal option by increasing the price limitation by $16,324 from $8,162 to $24,486 and by extending the completion date from June 30, 2022 to June 30, 2024. #25      Authorize to enter into a sole source amendment to an existing contract with NH Coalition Against Domestic and Sexual Violence, Concord, NH, (originally approved by G&C on 6/10/15, item #11),  for the continued provision of a statewide program to improve responses and support to victims of domestic violence and their children, by increasing the price limitation by $2,944,472 from $8,568,853 to $11,513,325 and by extending the completion date from June 30, 2024 to June 30, 2025.                         Division of Economic & Housing Stability
#26      Authorize to enter into a contact with the Pyramid Model Consortium, Missoula, MT, in the amount of $450,000 for Professional Development Services for Inclusive Practices and Social Emotional Development in Early Childhood Care and Education. Effective upon G&C approval or July 1, 2022 whichever is later, through June 30, 2024, with the option to renew for up to 4 additional years. 100% Federal Funds.

#27      Authorize to enter into a sole source amendment to an existing contract with Care Solutions, Inc., Atlanta, GA, (originally approved by G&C on 4/5/17, late item #A), to continue to provide a web-based Health and Safety training program and ongoing support and maintenance, by increasing the price limitation by $195,673 from $141,500 to $337,173 and by extending the completion date from June 30, 2022 to June 30, 2025. Effective upon G&C approval or July 1, 2022 whichever is later. 100% Federal Funds.

#28      Authorize the Bureau of Turnpikes to transfer $15,000 between various classes in Turnpike’s renewal and replacement. Effective upon G&C approval through June 30, 2022. 100% Turnpike funds. Contingent upon Fiscal Committee approval on May 20, 2022.

#32      Authorize the Bureau of Right-of-Way to pay property owners, tenants and/or claimants $35,112.87 as documented in the Contemplated Awards List, for amounts greater than $5,000 for the period extending from April 26, 2022 through May 4, 2022. Effective upon G&C approval.

#33      Authorize the Bureau of Right-of-Way to sell a 1.89+/- acre parcel of state-owned land with improvements, located at 1556 White Mountain Highway in the Town of Madison. The sale will be to A.J. Coleman & Sons Inc. and/or assigns (Grantee) for $301,000, plus a $1,100 administrative fee. Effective upon G&C approval. (2) Further authorize to compensate Coldwell Banker Lifestyles from the proceeds of the sale in the amount of $15,050.

#34      Authorize the Bureau of Construction to enter into a contract amendment with R.M. Piper, Inc., Plymouth, NH, (originally approved by G&C on 7/14/21, item #22),  by increasing the contract amount by $300,000, from $1,567,526 to $1,867,526, for bridge preservation work for 3 bridges located in the Towns of Meredith and Gilford. Effective upon G&C approval. 100% Federal Funds.

#35      Authorize the Bureau of Rail and Transit to pay the City of Nashua, Nashua Transit System, Nashua, NH, an amount not to exceed $69,564, to provide a portion of the non-Federal Funds required to match Federal Transit Administration operating funds. Effective upon G&C approval through June 30, 2023. 100% General Funds.

#36      Authorize the Bureau of Highway Design to enter into an Agreement with Haley & Aldrich Inc., Bedford, NH, for an amount not to exceed $400,000 for on-call engineering services for various transportation projects located throughout the State. Effective upon G&C approval through April 30, 2025.          

#37      Authorize the Bureau of Construction to enter into a contract with All States Construction, Inc., Sunderland, MA, on the basis of a single bid of $1,739,805.45 for resurfacing of 10.2 miles if pavement on 6 sections in 6 Towns in Districts 4 and 5 including Rindge, Mont Vernon, Dunbarton, Allenstown, Warner, and Hopkinton. (2) Further authorize a contingency in the amount of $86,990.27 for payment of latent conditions, which may appear during the construction of the project. Effective upon G&C approval through October 14, 2022. 100% Other Funds (SB-367).

#38      Authorize the Bureau of Construction to enter into a contract with Busby Construction Co. Inc., Atkinson, NH, on the basis of a low bid of $574,955 for the rehabilitation of a metal culvert carrying Bowman Brook under NH 114 in Bedford. (2) Further authorize a contingency in the amount of $57,495.50 for payment of latent conditions, which may appear during the construction of the project. Effective upon G&C approval through October 28, 2022. 100% Federal Funds.

#39      Authorize the Bureau of Construction to enter into a contract with The Gorman Group, LLC d/b/a GORTECH, Albany, NY, on the basis of a low bid of $4,796,762.26 for pavement preservation on I-93 in Woodstock and Lincoln, in District 3. (2) Further authorize a contingency in the amount of $239,838.11 for payment of latent conditions, which may appear during the construction of the project. Effective upon G&C approval through September 16, 2022. 100% Federal Funds.

#40      Authorize the Bureau of Transportation Systems, Management & Operations to enter into a Tri-State Agreement with the Maine Department of Transportation and the Vermont Agency of Transportation for the New England Compass Advanced Transportation Management System and Traveler Information System. Effective upon G&C approval. (2) Further authorize to enter into a sole source contract with Southwest Research Institute, San Antonio, TX, in the amount of $10,129,071.94 for the purpose of providing an Advanced Transportation Management Software – New England Compass. Effective upon G&C approval through April 30, 2026 with an option to extend, at the sole discretion if the State, for up to 3 additional 2 year extension periods up to, but not beyond April 30, 2032. 35.8% Federal, 64.2% Other Funds (States of Maine and Vermont).

                        DEPARTMENT OF INFORMATION TECHNOLOGY
#41      Authorize, on behalf of the Department of Revenue to amend a contract with Innovative Technical Consulting, Inc., Suwanee, GA, (originally approved by G&C on 12/20/17, item #43), by exercising a contract renewal option and by increasing the price limitation by $18,000 from $150,000 to $168,000, and by extending the completion date from June 30, 2022 to December 31, 2022 for the purpose of providing comprehensive technical support services for the IBM Power 8 Server. Effective upon G&C approval through December 31, 2022. 100% Other Funds.

#42      Authorize, on behalf of the Department of labor, to enter into a sole source contract with Innovative Technical Consulting Inc., Atlanta, GA, in the amount not to exceed $54,000 for the purpose of providing technical support necessary to maintain the on premise IBM AS400 Power Servers in support of the NH Department of Labor. Effective upon G&C approval through June 30, 2025, with the option to extend up to 3 additional years. 100% Other Funds.

                        NEW HAMPSHIRE HOUSING APPEALS BOARD
#43      Authorize a retroactive payment to Steven B. Keach, in the amount of $1,050 for fourteen days worked between March 24, 2021 through June 30, 2021. Effective upon G&C approval. 100% General Funds.

                        OFFICE OF PROFESSIONAL LICENSURE AND CERTIFICATION
#44      Authorize to transfer $118,500 between classes and to establish class 103, to support the completion of several renovations for the leased space at 7 eagle Square and vehicle acquisition.                         COMMUNITY DEVELOPMENT FINANCE AUTHORITY
#45      Authorize under the Community Development Block Grant program, to award a grant to the County of Cheshire, Keene, NH, in the amount of $500,000 to support the public facilities rehabilitation project situated within the Harper Acres affordable housing development. Effective upon G&C approval through December 31, 2024. 100% Federal Funds.

#107     Authorize the Division of Plant and Property to make a one-time retroactive payment to DAS employee Kevin J. Jordan in the amount of $1,667.03 for work performed pursuant to a temporary promotion that was in effect for the period of March 12, 2021 through June 30, 2021. Effective upon G&C approval. 100% Transfers from Other Agencies.

#108     Authorize the approval of an annual salary step increase for Stephen Mason, education Training Officer in the Division of Personnel, from Grade DD, Step 4,  Salary $76,830, Grade DD, Step 5, Salary $81,042. Effective April 22, 2022.

#109     Authorize to partially terminate on behalf of the State of NH, that a certain 99-year lease dated April 1, 2000, by and between the State and the City of Laconia, with regard to 2 parcels of State land located in the City of Laconia; further authorize the Department to transfer, on behalf of the State, one of said previously leased parcels of land to the City of Laconia, said transfer to be made in exchange for: (i) the City’s cooperation in partially terminating said lease, (ii) the City’s abandonment and extinguishment of certain unused easements in gross encumbering the State’s title to the main Lakes Region Facility campus tract, and (iii) the City’s termination and release of a certain unused license associated with the unused easements and also certain State covenants of record purporting to restrict certain capabilities and activities at the former Lakes Region Correctional Facility previously located on the LRF campus.

#110     Authorize the Bureau of Court Facilities to enter into a sole source nine month lease agreement with the Bar Harbor Bank & Trust, Bar Harbor, ME, for an amount not to exceed $46,125, which will provide courtroom and office space comprised of 4,191 square feet for the 6th Circuit-District Division-Hillsborough Courthouse. Effective upon G&C approval for the period of June 1, 2022 through February 28, 2023. 100% Transfer Funds (Transfer from AOC, Rent from Other Agencies).

#111     Authorize to enter into a contract with Bouchard Painting, Inc., Derry, NH, for a total price not to exceed $175,000 for interior and exterior painting services at several state owned buildings managed by the Department. Effective upon G&C approval or May 18, 2022, whichever is later, through May 17, 2024, with an option to renew for an additional year.

#112     Authorize to enter into a contract with Arthur St. Laurent d/b/a Saints Lawn Care, Pittsfield, NH, for a total price not to exceed $50,000 for landscaping services. Effective upon G&C approval or May 18, 2022, whichever is later, through May 17, 2024, with an option to renew for an additional year.

#113     Authorize to enter into a contract with Katahdin Properties Services LLC, Dover Foxcroft, ME, for total price not to exceed $135,000 for landscaping services. Effective upon G&C approval or May 18, 2022, whichever is later, through April 19, 2024, with an option to renew for an additional year.

#114     Authorize to amend a contract with Consolidated Communications Enterprise Services, Inc., d/b/a Consolidated Communications, Mattoon IL, (originally approved by G&C on 5/3/17, item #104), for Carrier Ethernet Services by increasing the price limitation by $2,000,000 from $2,800,000 TO $4,800,000 and by extending the completion date from May 31, 2022 to May 31, 2024.
