#6        Authorize to accept and expend federal funds from the American Rescue Plan Act in the amount of $500,500 which will be used to fund a third-party vendor to review and consult on how to improve the Medicaid Care Management model.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022. 

#7        Authorize to amend an accept and expend item (Informational Item #T, from the G&C meeting on May 19, 2021), to modify and reallocate the budget by transferring funds in the amount of $82,537 between classes.  Effective upon G&C approval through June 30, 2023.  Contingent upon Fiscal Committee approval on January 21, 2022. 

#8        Authorize to enter into an Asset Purchase agreement with Hampstead Outlook Inc., Philips J. Kubiak, as Trustee of the Trusts Named Herein, in the amount of $13,000,000 and execute all necessary closing documents, for the purchase of a 111-bed, licensed and accredited psychiatric hospital for children and young adults, in Hampstead, NH, and acquisition of all assets, rights, and interests in the business.  Effective upon G&C approval.  100% Federal Funds. 

#9        Authorize to accept and expend federal funds from the American Rescue Plan Act in the amount of $15,015,000 which will be used to provide capital defrayment to Portsmouth Regional Hospital and its parent company HCA Healthcare Inc., to develop a consolidated behavioral health hospital.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022.

#13      Authorize the determination made by the Commissioner that on December 9, 2021, a Mental Health Worker II at NH Hospital sustained an injury in the line of duty due to a hostile or overt act, or an act caused by another during the performance of duties which are considered dangerous in nature.  Effective upon G&C approval.

#15      Authorize to accept and expend funds from the Centers for Disease Control and Prevention to fund the Public Health Crisis Response Workforce Development program in the amount of $8,952,425.  (2)Further authorize to create two new positions for COVID-19 Workforce Development purposes.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Continent upon Fiscal Committee approval on January 21, 2022.

#18      Authorize to enter into a contract with the NH Coalition Against Domestic and Sexual Violence, Concord, NH, for the provision of primary prevention activities for the reduction of sexual violence, in the amount of $599,873.  Effective February 1, 2022 through January 31, 2024.  100% Federal Funds. 

#19      Authorize to enter into a contract with Appriss Inc., Louisville, KY, for a prescription monitoring solution that provides real-time data concerning the dispensing of controlled drugs to assist practitioners in ensuring controlled medications are appropriately prescribed and dispensed, in the amount of $2,535,750.  Effective upon G&C approval through June 30, 2026.  100% Federal Funds. 

#19A    Authorize to amend an existing contract with Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 12-8-21, late item A), for the purpose of adding additional temporary medical staff to assist with the increase in COVID-19 cases, by increasing the price by $2,000,000 from $6,000,000 to $8,000,000 with no change to the completion date of June 30, 2022.  100% Federal Funds.

#20      Authorize the Bureau of Housing Supports to accept and expend the Youth Homelessness Demonstration Grant award in the amount of $110,000.  (2)Further authorize to establish one full-time temporary Program Specialist IV, LG 25.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Continent upon Fiscal Committee approval on January 21, 2022. 

#21      Authorize to amend an existing contract with Community Action Partnership of Strafford County, Dover, NH (originally approved by G&C on 6-24-20, item #21), to continue providing a Coordinated Entry Domestic Violence Program to individuals and families who are fleeing, or are attempting to flee, domestic violence, and who are at risk of homelessness, or experiencing homelessness, by increasing the price by $239,930 from $479,860 to $719,790 and by extending the completion date from October 31, 2022 to November 30, 2023.  100% Federal Funds. 

                        Division for Behavioral Health
#22      Authorize to amend item #29 originally approved by G&C on August 18, 2021, by transferring funds in the amount of $1,041,828 for the performance of work associated with the Substance Abuse Prevention & Treatment Block Grant and by extending the end date from March 14, 2023 to June 30, 2023.  100% Federal Funds.  Continent upon Fiscal Committee approval on January 21, 2022. 

#23      Authorize to amend item #28 originally approved by G&C on August 18, 2021, by transferring funds in the amount of $27,000 for the performance of work associated with the Emergency Grants to Address Mental and Substance Use Disorders during COVID-19.  Effective upon G&C approval through August 19, 2022.  100% Federal Funds.  Continent upon Fiscal Committee approval on January 21, 2022. 

#24      Authorize to enter into a sole source contract with Maxim Healthcare Staffing Services Inc., Manchester, NH, for pediatric psychiatric inpatient care strike teams, in the amount of $2,223,977.  Effective January 26, 2022 or upon G&C approval, whichever is later, through July 31, 2022, with the option to renew for up to two additional six month periods.  100% Federal Funds.  Continent upon Fiscal Committee approval on January 21, 2022. 

                        Division of Long Term Supports and Services
#25      Authorize to amend an accept and expend item of federal funds (Informational Item TT, G&C 6-30-21), to modify and reallocate the budget by transferring funds in the amount of $40,000 between classes.  Effective upon G&C approval through September 30, 2022.  100% Federal Funds (CRRSA 2021 APC5 Elder Justice Grant).  Continent upon Fiscal Committee approval on January 21, 2022. 

#26      Authorize to enter into a contract with Myers and Stauffer LC, Kansas City, MO, for rate build up rate setting services, as applicable to the 1915(c) Home and Community-Based Services Waivers program, in the amount of $405,609.  Effective upon G&C approval through November 30, 2023, with the option to renew for up to 2 additional years.  50% General, 50% Federal Funds. 

                        Division for Children, Youth & Families
#27      Authorize to enter into a retroactive sole source amendment to an existing contract with NH Coalition Against Domestic and Sexual Violence, Concord, NH (originally approved by G&C on 6-10-15, item #11), for the continued provision of a statewide program to improve responses and support to victims of domestic violence and their children, by increasing the price by $478,136 from $8,090,717 to $8,568,853 and by extending the completion date from June 30, 2023 to June 30, 2024.  Effective December 1, 2021.  100% Federal Funds. 

                        DEPARTMENT OF TRANSPORTATION
#28      Authorize to pay membership dues retroactively in the amount of $31,365.73 to the American Association of State Highway and Transportation Officials, Washington, DC, for continued membership in the national transportation organization.  Effective January 1, 2022 through December 31, 2022.  18% Federal, 76% Highway, 6% Other (Agency Income) Funds. 

#29      Authorize to enter into an extended maintenance agreement with the City of Berlin for the continued maintenance of a section of NH 110 for a period of 5 years following the reclassification of that section to a Class IV highway under the City of Berlin’s jurisdiction and ultimate maintenance responsibility.  Effective upon G&C approval.

#30      Authorize the Bureau of Finance and Contracts to pay a small claim in the amount of $831.30.  Effective upon G&C approval.  100% Highway Funds. 

#31      Authorize the Bureau of Finance and Contracts to pay small claims in the amount of $1,956.43.  Effective upon G&C approval.  100% Highway Funds. 

#32      Authorize to accept and place on file, the Bureau of Right of Way’s Contemplated Awards List for amounts less than $5,000 for the sum total of $11,479 for the period extending from October 1, 2021 through December 31, 2021. 

#33      Authorize the Bureau of Turnpikes to retroactively accept and expend revenue in the amount of $13,967 from the City of Rochester for the construction of a sidewalk located off Ten Rod Road in Rochester, NH.  Effective upon G&C approval through June 30, 2022.  100% Private/Local Funds. 

#34      Authorize the Bureau of Rail & Transit to accept and expend Agency Income in the amount of $110,000 from the Friends of the Northern Rail Trail for the purpose of acquiring a portion of the Northern Railroad Corridor.  Effective upon G&C approval through June 30, 2023.  100% Agency Income.  Continent upon Fiscal Committee approval on January 21, 2022. 

#35      Authorize the Bureau of Rail & Transit to enter into a sole source contract amendment with Concord Coach Lines Inc., Concord, NH (Informational Item D, G&C 1-3-21), to support daily commuter bus service between Concord, NH and Boston, MA, by increasing the amount by $800,000 from $1,891,125 to $2,691,125.  Effective upon G&C approval through June 30, 2022.  100% Federal Funds. 

#36      Authorize the Bureau of Planning & Community Assistance to retroactively amend an agreement with the Town of Sanbornton, sub-recipient (originally approved by G&C on 10-29-14, item #25), to provide funding and services under the State Highway Aid Program through a Local Project Administration process, by increasing the amount by $1,528,226.33 from $120,000 to $1,648,226.33.  Effective upon G&C approval through June 30, 2022.  100% State Highway Aid funds. 

#37      Authorize the Bureau of Highway Design to enter into agreements associated with the design and construction of a new bridge (NH Br. No. 168/152; Maine Br. No. 3717) carrying the New Bridge Road and Townhouse Road in the towns of Lebanon, ME and Milton, NH, over Northeast Pond as detailed in letter dated November 10, 2021.

#38      Authorize the Bureau of Rail & Transit to amend an existing contract with RLS & Associates Inc., Dayton, OH (originally approved by G&C on 5-15-19, item #31), for continued administration of the NH Rural Transit Assistance program, by exercising a contract renewal option, by increasing the price by $200,000 from $579,955 to $779,955, and by extending the completion date from March 31, 2022 to March 31, 2023.  100% Federal Funds. 

#39      Authorize the Bureau of Materials & Research to enter into a sole source agreement with the NorthEast Transportation Training and Certification Program Inc., Winchendon, MA, to participate in NETTCP’s training and certification program, for a fee not to exceed $160,000.  Effective upon G&C approval through December 31, 2026, with the option to renew for an additional 2 year period.  100% Federal Funds. 

#40      Authorize the Bureau of Rail & Transit to enter into a contract with the NH Northcoast Corp., Ossipee, NH, for track improvements on its privately-owned short line railroad, that runs between Rollinsford and Ossipee, NH in Strafford and Carroll Counties, for an amount not to exceed $250,000.  Effective upon G&C approval through September 30, 2024.  100% Capital Funds. 

#41      Authorize the Bureau of Bridge Design to enter into an agreement with the firm of GM2 Associates Inc., Concord, NH, for Part “A” preliminary design services, environmental coordination and public involvement support efforts, and land surveying services, for an amount not to exceed $523,631.44.  The project proposes to rehabilitate NH Route 16 bridges which carry Northbound and Southbound traffic of the Spaulding Turnpike over the Cocheco River in the City of Dover, NH.  The bridges are on the Department’s Red List of deficient structures.  Effective upon G&C approval through June 30, 2024.  100% Turnpike Funds. 

#42      Authorize the Bureau of Construction to enter into a contract with Continental Paving Inc., Londonderry, NH, for pavement preservation in Districts 4 and 5, on the basis of a low bid of $3,954,653.75.  (2)Further authorize a contingency in the amount of $197,732.69 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through September 23, 2022.  100% Federal Funds. 

#43      Authorize the Bureau of Construction to enter into a contract with Evroks Corporation, Northfield, NH, for bridge rehabilitation in Manchester of Huse Road over I-293 and NH Rt. 101, on the basis of a single bid of $3,070,680.10.  (2)Further authorize a contingency in the amount of $307,068.01 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through November 18, 2022.  100% Federal Funds. 

#44      Informational - The Bureau of Construction entered into a contract with Casella Construction of Mendon, VT, for roadway and slope stabilization on NH Rt 12 in Charlestown, on the basis of a low bid of $2,638,500.50, based on G&C approval on November 22, 2021 late item B.  (2)A Contingency in the amount of $263,850.05 for payment of latent conditions which may appear during the construction of the project is also included.  100% Federal Funds. 

#45      Authorize the Bureau of Construction to enter into a contract with Continental Paving Inc., Londonderry, NH, for resurfacing of four sections in District 6, on the basis of a low bid of $5,026,190.20.  (2)Further authorize a contingency in the amount of $251,309.51 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through September 23, 2022.  100% Federal Funds. 

#46      Authorize the Bureau of Turnpikes to retroactively amend the contract with Kapsch TrafficCom USA Inc., with principal offices in McLean, VA (originally approved by G&C on 5-9-12, item #147), to provide E-ZPass Group a reduction in pricing of E-ZPass related equipment; the addition of ISO 18000-6 Type C sticker windshield tags; and the extension of the exclusivity.  Effective November 1, 2021 to November 1, 2022.  100% Turnpike Funds. 

#47      Authorize the Bureau of Construction to enter into a contract with C.W. Sliter & Sons LLC d/b/a CWS Fence & Guardrail Co., Andover, NH, for guardrail replacement in District 1, on the basis of a low bid of $1,059,057.84.  (2)Further authorize a contingency in the amount of $52,952.89 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through September 23, 2022.  100% Federal Funds. 

#48      Authorize the Bureau of Construction to enter into a contract with R.M. Piper Inc., Plymouth, NH, for bridge replacement in Bedford, on the basis of a sole bid of $5,428,180.50. (2)Further authorize a contingency in the amount of $271,409.03 for payment of latent conditions which may appear during the construction of the project.    Effective upon G&C approval through July 14, 2023.  100% Federal Funds. 

                        STATE TREASURY
#49      Authorize to proceed with the sale and issuance of up to $80 million of general obligation (“new money”) bonds through a competitive process, negotiated sale, or private placement.  This resolution will have an expiration date of January 31, 2023.  (2)Further authorize the State Treasurer to conduct the sale and issuance of up to $55 million of general obligation refunding bonds through a competitive process, negotiated sale, or private placement to refund outstanding general obligation bonds of the State to generate net present value debt service savings.  This resolution will also have an expiration date of January 31, 2023.

#50      Authorize the issuance of up to $90 million of Turnpike System Revenue Bonds, 2022 Refunding Series (the “Bonds”) pursuant to RSA 237-A.  The State Treasurer is permitted to proceed with the sale and issuance of these Bonds through a competitive process, negotiated sale, or private placement. 

                        DEPARTMENT OF MILITARY AFFAIRS AND VETERANS SERVICES
#51      Authorize to enter into a sole source contract amendment with JBC Construction LLC, Londonderry, NH, for renovation of the latrine areas in Building C on the State Military Reservation, by adding a contingency line in the amount of $20,000 and by extending the end date from February 28, 2022 to May 31, 2022.  50% Federal, 50% General Funds. 

                        DEPARTMENT OF AGRICULTURE, MARKETS & FOOD
#52      Authorize the Division of Agricultural Development to accept and expend $398,133 from the US Department of Agriculture to conduct programs benefiting NH for the Specialty Crop Block Grant Program.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022. 

                        INSURANCE DEPARTMENT
#53      Authorize an annual salary increase for David J. Bettencourt, Deputy Commissioner, in the amount of $5,616 from a current salary of $107,952 to a new annual salary of $113,568.  Effective upon G&C approval.  100% Agency Income. 

#54      Authorize to place Tyler Brannen at Step 7 for the position of Life and Health Director, unclassified salary grade HH, earning $119,184.  Effective upon G&C approval.  100% Agency Income.

                        NEW HAMPSHIRE EMPLOYMENT SECURITY
#56      Authorize to accept and expend $1,899,145 in additional federal Unemployment Insurance funds from the US Department of Labor – Employment and Training Administration resulting from the significant increase in unemployment claims due to COVID-19.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022. 

                        DEPARTMENT OF CORRECTIONS
#57      Authorize to accept and expend $539,490 of American Rescue Plan Act of 2021, State and Local Fiscal Recovery Funds, for the purpose of utilizing the National Guard to provide emergent staffing needs.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022. 

#58      Authorize to accept and expend $1,321,200 of American Rescue Plan Act of 2021, State and Local Fiscal Recovery Funds, for the purpose of replacing antiquated fiber optic cabling with modern fiber armored cabling at the NH State Prison for Men, Concord.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022. 

#59      Authorize to accept and expend $7,156,500 of American Rescue Plan Act of 2021, State and Local Fiscal Recovery, for the purpose of upgrading the offender management system.  Effective upon G&C approval through June 30, 2023.  100% Federal Funds.  Contingent upon Fiscal Committee approval on January 21, 2022.

#71      Authorize to enter into a sole source agreement with Lamprey Health Care, Newmarket, in an amount not to exceed $782,100 in American Rescue Plan Act State Fiscal Recovery Funds, to fund the NH Needs Caregivers Licensed Nursing Assistants Extended Learning Opportunities Program to rapidly grow a pipeline of caregivers available for work in NH’s nursing homes and other healthcare settings by piloting a program to recruit and train at least 300 high school students to become LNAs.  Effective upon G&C approval through December 31, 2022.  100% Federal Funds. 

#72      Authorize to enter into a sole source amendment to an existing sub award (or grant) agreement with NH Housing Finance Authority, Bedford, NH (originally approved by G&C on 9-15-21), to provide emergency rental assistance and housing stability services to eligible NH households that have lost income or experienced other financial hardship because of the COVID-19 pandemic by increasing the price by $5,000,000 from $40,000,000 to $45,000,000.  Effective upon G&C approval through September 30, 2025.  100% Federal Funds. 

#73      Authorize to enter into a sole source amendment to an existing sub award (or grant) agreement with NH Housing Finance Authority, Bedford, NH (originally approved by G&C on 6-30-21), for program planning and administration of Homeowner Assistance Fund, under which funds will be used to mitigate financial hardships suffered by NH residents associated with COVID-19 pandemic by providing funds to prevent displacements of homeowners, by increasing the price by $1,250,000 from $3,000,000 to $4,250,000.  Effective upon G&C approval through September 30, 2025.  100% Federal Funds. 

#73A    Authorize to accept and place on file the list of entities per program and awards of these CARES Act Funds awarded.  Effective upon G&C approval.  100% Federal Funds.
