#12A      Authorize to enter into retroactive sole source agreements with the vendors as detailed in letter dated February 9, 2018, for the provision of case management services as well as culturally and linguistically appropriate activities to NH refugees 60 years of age or older in order to promote community integration, naturalization, health and mental health, in an amount not to exceed $100,000.  Effective December 1, 2017 through September 30, 2019.  100% Federal Funds.   
 

                                Division of Behavioral Health
#12B      Authorize the Bureau of Drug and Alcohol Services to enter into a retroactive sole source amendment with Granite Pathways, c/o Fedcap, Concord, NH (originally approved by G&C on 7-13-16, item #6C), to continue Regional Access Point Services that assist individuals with substance use disorders obtain the services, by increasing the price by $100,000 from $1,998,587 to $2,098,587.  Effective February 1, 2018 through the original end date of June 30, 2018.  80% Federal, 20% General Funds. 

                                DEPARTMENT OF TRANSPORTATION 
#13          Authorize the Division of Operations to transfer $6,245,000 from Highway Surplus to fund winter maintenance activities.  Effective upon G&C approval through June 30, 2018.  100% Highway Funds.  Contingent upon Fiscal Committee approval on February 16, 2018.   
 

#14          Authorize the Bureau of Highway Design to budget and expend prior year carry forward funds in the amount of $214,962 to further fund the wildflower program.  Effective upon G&C approval through June 30, 2018.  100% Agency Income.  Fiscal Committee approved.    

#15          Authorize the Bureau of Construction to enter into an agreement with WSP USA Inc., Manchester, NH, to provide on-call construction engineering and inspection services for various transportation projects statewide, in the amount of $1,950,000.  Effective upon G&C approval through March 31, 2021.    

#16          Authorize the Bureau of Construction to enter into a contract with R.S. Audley Inc., Bow, NH, for replacement of the bridge carrying Mill Street over NHRR in the town of Haverhill, on the basis of a low bid of $1,176,845.50.  (2)Further authorize a contingency in the amount of $58,842.28 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through October 26, 2018.  100% Federal Funds.    

#17          Authorize the Bureau of Construction to enter into a contract with Pike Industries Inc., Belmont, NH, for pavement preservation on US 302/NH 16, from the end of the median island on NH 16 south of the NH 16/US 302 intersection northerly to a pavement joint just south of the Bartlett/Conway Town line, on the basis of a low bid of $1,891,838.  (2)Further authorize a contingency in the amount of $94,591.90 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through June 22, 2018.  100% Federal Funds.    

#18          Authorize the Bureau of Planning and Community Assistance to enter into a contract with the Central NH Regional Planning Commission, Concord, NH, to undertake and update the Coordinated Public Transportation Plan for the Midstate Region, in the amount of $21,300.30.  Effective upon G&C approval through June 30, 2019.  100% Federal Funds.    

#19          Authorize the Bureau of Construction to enter into an agreement with Greenman-Pedersen Inc., Portsmouth, NH, to provide on-call construction engineering and inspection services for various transportation projects statewide, in the amount of $1,950,000.  Effective upon G&C approval through March 31, 2021.  

#20          Authorize the Bureau of Planning and Community Assistance to enter into a Tri-State agreement with Vermont and Maine to formalize the overall goals and basic tenets to develop, maintain, and improve the shared software known as MATS (Managing Assets for Transportation Services).  Effective upon G&C approval through December 31, 2022.  (2)Further authorize to enter into an agreement with the Vermont Agency of Transportation, Montpelier, VT for the maintenance management and functionality enhancements for Managing Assets for Transportation Systems, the shared software with VT and ME known as MATS, for a total fee not to exceed $400,000.  Effective upon G&C approval through June 30, 2019.  100% Other – Transfer to DoIT).    

#21          Authorize the Bureau of Highway Maintenance, Well Section, to enter into a contract with Capital Well Company Inc., Dunbarton, NH, for a 6-inch drilled well and pump on the property of John Stanhope, Orford, NH, in the amount of $22,005.  Effective upon G&C approval through June 29, 2018.  100% Highway Funds.    

#22          Authorize the Bureau of Highway Maintenance, Well Section, to enter into a contract with Skillings & Sons Inc., Amherst, NH, for installation of a new 4” casing with jaswell seal in the existing well and pump with ancillary equipment on the property of Richard Giard, Deerfield, NH, in the amount of $13,714.50.  Effective upon G&C approval through June 29, 2018.  100% Highway Funds.    

#23          Authorize the Bureau of Construction to enter into a contract with Bazin Brothers Trucking Inc., Westminster, VT, for preservation efforts to stabilize bridges from scour, on the basis of a low bid of $994,570.  (2)Further authorize a contingency in the amount of $69,619.90 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through November 1, 2019.  100% Federal Funds.    

#24          Authorize the Bureau of Construction to enter into a contract with CPM Constructors Inc., Freeport, ME, for furnishing and installing debris shielding and or debris netting on 15 bridges statewide, on the basis of a low bid of $1,301,159.90.  (2)Further authorize a contingency in the amount of $65,058 for payment of latent conditions which may appear during the construction of the project.  Effective upon G&C approval through September 14, 2018.  91% Federal, 9% Turnpike Funds.

                                COMMUNITY DEVELOPMENT FINANCE AUTHORITY 
#25          Authorize to award a grant to the Town of Winchester, NH, in the amount of $500,000 to support the Jones Road infrastructure improvement project.  Effective February 21, 2018 through December 31, 2019.  100% Federal Funds.

#32          Authorize to enter into a contract with ExpressMed LLC d/b/a Orchard Medical Management, Manchester, NH, for the provision of Pre-Assignment & Fitness for Duty Exam Services, in the amount of $42,635.  Effective upon G&C approval through June 30, 2020, with the option to renew for one additional period of up to two years.  100% General Funds.    

#33          Authorize to retroactively amend a holdover lease agreement in an amount not to exceed $9,685.68 with Bergeron Construction Company Inc., Keene, NH, for the purpose of probation/parole office space for the Keene Probation & Parole District Office for the period of February 3, 2018 through June 2, 2018.  100% General Funds.

Davis Construction Company LLC, Peterborough, NH, for construction of 9 accessible pit toilets at 7 state parks, in the amount of $145,384.  Effective upon G&C approval through June 29, 2018.  100% Capital Funds.    

#37          Authorize the Bureau of Historic Sites to enter into a contract with Avatar Construction Corporation, Cambridge, MA, to perform roof replacement on Wentworth Coolidge Mansion in Portsmouth, in the amount of $168,600.  Effective upon G&C approval through October 31, 2018.  100% Capital Funds.    

                                DEPARTMENT OF ENVIRONMENTAL SERVICES 
#38          Authorize to retroactively accept and expend $625,824 in Drinking Water and Groundwater Trust Funds for the purpose of implementing RSA 485-F.  Effective October 1, 2017 through June 30, 2018.  (2)Further authorize to establish five full time temporary positions through June 30, 2019.  100% Drinking Water and Groundwater Trust Funds.  Contingent upon Fiscal Committee approval on February 16, 2018.   
 

#39          Authorize to award a sole source agreement to the Great Bay Stewards Inc., Greenland, NH, in the amount of $18,040 to conduct outreach and training associated with living shoreline stabilization projects.  Effective upon G&C approval through September 30, 2020.  100% Federal Funds.    

#40          Authorize to execute a sole source amendment to the contract with Pepperrell Cove Marine Services Inc., Kittery, ME (originally approved by G&C on 7-22-15, item #86), to perform moorings services associated with oil spill containment strategies and spill response boats, by increasing the amount by $20,000 to $100,000 from $80,000, and by extending the expiration date to June 30, 2020 from June 30, 2019.  100% Oil Pollution Control Funds.    

#41          Authorize to execute a sole source amendment to the contract with Great Bay Marine Inc., Newington, NH (originally approved by G&C on 6-15-16, item #51), to provide dock slips, launching rights and storage space for six NHDES emergency response boats, by increasing the amount by $40,000 from $120,000 to $160,000, and by extending the expiration date to June 30, 2020 from June 30, 2019.  100% Oil Pollution Control Funds.    

#42          Authorize to retroactively pay the fiscal year 2018 membership dues to the Northeast Environmental Enforcement Project, Delaware, OH, in the amount of $4,000 for the period of July 1, 2017 through June 30, 2018.  38% Federal, 62% Hazardous Waste Funds.    

#43          Authorize to award a Cyanotoxin Monitoring Equipment and Training grant to the City of Rochester Water Department, Rochester, NH, in the amount of $5,980 to purchase equipment to monitor for cyanobacteria at their public drinking water supply sources.  Effective upon G&C approval through November 30, 2018.  100% Drinking Water SFR Repayment Funds.    

#44          Authorize to retroactively award a grant to the Town of Northumberland, NH, in the amount not to exceed $200,000 for water system improvements.  Effective November 3, 2017 through December 1, 2018.  100% Drinking Water/Groundwater Trust Funds.    

#45          Authorize to retroactively amend an agreement with the Town of Wolfeboro, NH (originally approved by G&C on 5-27-15, item #101), for the Rust Pond Watershed Restoration Plan Implementation Phase 2 – Sites 1, 3 and 4 BMPs project by extending the completion date to December 31, 2018 from December 31, 2017.  No Additional Funding.  100% Federal Funds.    

#46          Authorize to award a grant to the Bethlehem Village District, Bethlehem, NH, in the amount not to exceed $750,000 for water system improvements.  Effective upon G&C approval through June 1, 2019.  100% Drinking Water/Groundwater Trust Funds.    

#47          Authorize to award a sole source agreement to the University of NH, Durham, NH, to conduct outreach and training associated with living shoreline stabilization projects, in the amount of $97,467.  Effective upon G&C approval through September 30, 2020.  100% Federal Funds.

#54          Authorize to enter into a contract with Paradigm Consulting LLC, to provide professional development, technical assistance, support, and resources to school districts and families on screening, identification of risk factors for dyslexia and related disorders, and intervention strategies, in an amount not to exceed $88,980.  Effective upon G&C approval through October 30, 2018.  100% General Funds.

#56          Authorize to enter into a contract with Nancy Brigham Associates, in an amount not to exceed $27,636.24 to assist the department’s E3 Teen Fatherhood Program in the area of program evaluation, in an amount not to exceed $27,636.24.  Effective upon G&C approval through June 30, 2018.  100% Federal Funds.    

#56A      Authorize to make an unencumbered stipend payment to an applicant for travel expenses related to an interview for the position of Attorney III, with the department’s search committee in an amount not to exceed $1,500.  Effective upon G&C approval through June 30, 2018.  100% General Funds.

#57          Authorize the Office of Highway Safety to enter into a retroactive sole source contractual agreement with the University of NH, Durham, NH, to coordinate a highway safety impaired driving related advertisement campaign during the 2017/2018 season, in the amount of $63,000.  Effective October 1, 2017 through September 30, 2018.  100% Federal Funds.   
 

#58          Authorize to retroactively pay Brian Hester the amount of $1,646.25 for 48 hours of accumulate annual leave owned him from April 1, 2016 through March 31, 2017.  Effective upon G&C approval.  49.11% General, 27.51% Turnpike, 23.84% Highway Funds.    

#59          Authorize the Division of Homeland Security and Emergency Management to enter into a memorandum of agreement with the Division of State Police to renew the subscription and licensing fee for school emergency notification software for 270 devices for a total amount of $24,675.  Effective upon G&C approval through September 30, 2018.  100% Federal Funds.  (2)Further authorize the Division of State Police to accept and expend $24,675 of federal funds from the Division of Homeland Security and Emergency Management for the renewal of the school emergency notification software.  Effective upon G&C approval through September 30, 2018.  100% Agency Income.    

                                DEPARTMENT OF JUSTICE 
#60          Authorize to enter into a sole source contract with Dr. Margaret Greenwald as a substitute medical examiner, for a total not to exceed $19,998, for the period of March 12-16, 2018 and April 18-21, 2018.  Effective upon G&C approval through June 30, 2018.  100% Federal Funds.   
 

#61          Authorize to enter into a sole source contract with Dr. Kristin Roman as a substitute medical examiner, for a total not to exceed $19,998, for the period of March 26-30, 2018 and April 9-13, 2018.  Effective upon G&C approval through June 30, 2018.  100% Federal Funds.    

#62          Authorize to enter into a subgrant with the Ossipee Police Department in the amount of $120,000 from the NH Regional Drug Task Force appropriation to support drug task force program operations.  Effective upon G&C approval through June 30, 2019.  100% Federal Funds.    

                                DEPARTMENT OF ADMINISTRATIVE SERVICES 
#63          Authorize the Division of Personnel’s request for waiver of classification decisions.  Effective upon G&C approval. 
 

#64          Authorize the Bureau of Purchase and Property to exercise a renewal option to extend a contract with New Horizons Computer Learning Centers d/b/a NH New Hampshire LLC of Nashua, NH (originally approved by G&C on 8-26-15, item #87), for the provision of professional technical training services, by increasing the amount by $75,000 from $300,000 to $375,000, and by extending the end date through June 30, 2020.    

#65          Authorize a new chapter of the Department of Administrative Services Manual of Procedures (“DAS MOP”), DAS MOP 151 (“Governor and Executive Council Policies”), effective upon G&C approval, and inclusion in that chapter of a policy identified as Governor and Executive Council Policy 2018-001.  The text of the proposed chapter and “Governor and Executive Council Policy 2018-001” is found in attachment A of the request.  

#66          Authorize the Bureau of Purchase and Property to retroactively exercise a contract amendment option with Arcomm Communications Corp, Hillsboro, NH (originally approved by G&C on 7-24-13, item #14), for the provision of Closed Cirucit Television maintenance and repair services.  This amendment represents the option to add locations and amend the scope of services to the contract and increase the price by $5,000 from $75,000 to $80,000 to account for the additional locations.  Effective August 1, 2016 through July 31, 2018.
